血府逐瘀汤联合利培酮、丙戊酸钠对感染性脑炎所致精神障碍伴攻击行为MOAS、PAN-SS评分的影响

摘　　要：目的：研究分析血府逐瘀汤联合利培酮、丙戊酸钠对感染性脑炎所致精神障碍伴攻击行为患者的临床治疗效果,以及对患者外显攻击行为量表（MOAS）、阳性与阴性症状量表（PAN-SS）评分的影响作用。方法：58例感染性脑炎所致精神障碍患者进行1-58编号,遵照随机分配原则,单数纳入对照组,双数纳入联合组,对照组应用利培酮联合丙戊酸钠治疗,联合组在此基础上加用血府逐瘀汤治疗,在治疗后1、3、6周应用MOAS以及PAN-SS量表对患者进行评价,分析不同治疗方法的临床疗效以及安全性。结果：共检出42株病原菌,其中革兰阳性菌27株,革兰阴性菌13株,真菌2株;两组患者治疗后1、3、6周MOAS评分与PANSS评分变化明显,联合组患者评分均低于对照组,组间数据比较结果有统计学意义（P 〈0. 05）;联合组患者治疗效果显著优于对照组,组间数据比较结果差异明显,有统计学意意义（P 〈0. 05）。结论：利培酮联合丙戊酸钠治疗感染性脑炎所致精神障碍并伴有攻击行为的患者有一定效果,但见效缓慢,联合血府逐瘀汤治疗有效提高了治疗效果,患者精神症状得到有效控制,是一种安全稳定的用药方法。Objective： To investigate the clinical therapeutic effect of Xuefu Zhuyu Decoction combined with risperidone and sodium valproate on patients with mental disorder with aggressive behavior caused by infection of encephalitis,and its impact on modified overt aggression scale（ MOAS） and positive and negative symptom scale（ PAN-SS）. Methods：Fifty-eight patients with mental disorders caused by infectious encephalitis were numbered from 1 ~ 58,and the single were put into the control group and even into the combination group according to the principle of random distribution.The control group was treated with risperidone combined with sodium valproate,and the combination group was treated with Xuefu Zhuyu Decoction on the basis of the control group. The patients were evaluated by MOAS and PAN-SS scale at 1,3 and 6 weeks after treatment. The clinical efficacy and safety of different treatment methods were analyzed. Results：A total of 42 pathogens were detected,including 27 Gram-positive bacteria,13 Gram-negative bacteria and 2 strains of fungi. After 1,3 and 6 weeks treatment,the MOAS score and PANSS score of two groups were significantly changed,and the scores of the combination group were lower than those of the control group,and the results of the comparison between the two groups were statistically significant（ P〈0. 05）. The treatment effect of the combination group was significantly better than that of the control group and the difference between the two groups was statistically significant（P〈0. 05）. Conclusion： Risperidone combined with sodium valproate has a certain effect in the treatment of patients with mental disorders caused by infectious encephalitis and associated with aggressive behavior,but the effect works slowly and the adverse reaction rate is high. The treatment effect effectively improves when combined with Xuefu Zhuyu Decoction. The patients＇ mental symptoms are effectively controlledand the incidence of adverse reactions is reducd,which is a safe and stable medication.
