平肝熄风汤联合拉贝洛尔对妊娠期高血压血液动力变化及妊娠结局的影响

摘　　要：目的：研究平肝熄风汤联合拉贝洛尔对妊娠期高血压患者的血液动力变化及妊娠结局的影响研究。方法：120例妊娠期高血压孕妇按随机数字法均分为对照组和观察组,各60例。对照组患者采用拉贝洛尔治疗;观察组采用平肝熄风汤联合拉贝洛尔治疗。记录妊娠期高血压患者治疗前、后舒张压、收缩压、平均动脉压及心率的变化情况,并对比两组患者的血液动力学及血糖变化情况。同时观察围生期婴儿不良状况发生率,如胎儿窘迫、胎儿生长受限、低体质量及新生儿窒息发生情况。结果：治疗前,两组患者的收缩压、舒张压、平均动脉压及心率无明显差别（P〉 0. 05）;治疗后,两组患者的收缩压、舒张压、平均动脉压及心率均明显降低,但观察组患者的收缩压、舒张压、平均动脉压及心率明显低于对照组,差异有统计学意义（P 〈0. 05）。治疗前,两组患者的血液动力学及血糖情况无明显差异（P〉 0. 05）;治疗后,两组患者的,血液动力学及血糖含量明显改善,但观察组的心脏指数（3. 19±0. 50） L/（min·m2）及血糖（5. 01±0. 49） mmol/L明显高于对照组心脏指数（3. 06±0. 47） L/（min·m2）及血糖（5. 09±0. 53） mmol/L,外周阻力（1. 25±0. 29）明显低于对照组（1. 36±0. 31）（P 〈0. 05）。治疗4周后,观察组的总有效率93. 34%（56/60）明显高于对照组81. 67%（49/60）（P 〈0. 05）。观察组不良妊娠结局发生率20. 00%（12/60）明显低于对照组60. 00%（P 〈0. 05）。结论：平肝熄风汤联合拉贝洛尔治疗妊娠期高血压,能有效改善患者血压及血液动力学,减少妊娠结局不良事件发生率,值得临床推广普及。Objective： To investigate the effect of Pinggan Xifeng Decoction combined with labetalol on the dynamic changes of blood in patients with hypertension and pregnancy outcome. Methods：A total of 120 cases of hypertensive disorder complicating pregnancy were randomly divided into the control group and the observation group,60 cases in each group. The control group were treated by rabello treatment and the observation group used Pinggan Xifeng Decoction combined with labetalol. The changes of diastolic blood pressure,systolic blood pressure,mean arterial pressure and heart rate before and after treatment were recorded,and the hemodynamic and blood glucose changes of the 2 groups were compared. At the same time,the incidence of adverse conditions in perinatal infants,such as fetal distress,fetal growth restriction,low body mass and neonatal asphyxia was observed. Results： Before treatment,two groups＇ systolic pressure,diastolic pressure and mean arterial pressure and heart rate had no significant difference（ P〉0. 05）. After treatment,two groups＇ systolic pressure,diastolic pressure and mean arterial pressure and heart rate were significantly decreased,but the observation group＇s systolic blood pressure,diastolic blood pressure,mean arterial pressure and the heart rate were significantly lower than those of the control group（ P〈0. 05）. Before treatment,there was no significant difference of hemodynamics or blood glucose in two groups（ P〉0. 05）. After treatment,two groups＇ hemodynamics and blood glucose levels were significantly improved,but the observation group＇s heart index（3. 19 ± 0. 50） L/（ min·m2） and blood glu-cose（5. 01 ± 0. 49） mmol/L were significantly higher than the control group＇s（ 3. 06 ± 0. 47） L/（ min · m2） and（5. 09 ± 0. 53） mmol/L and the peripheral resistance（1. 25 ± 0. 29） was significantly lower than the control group＇s（1. 36 ± 0. 31）（P〈0. 05）.
