摘 要:目的探讨磁共振血管成像(MRA)椎动脉颅内段发育变异的诊断价值。方法选择544例因缺血性脑卒中或短暂性脑缺血行颅脑MRA检查患者,其中男性276例,女性268例;年龄14~89岁,平均年龄51.86岁。行MRA。在Leonardo工作站进行后处理,分析颅脑MRA检查患者的图像,并观察椎动脉发育变异情况。按椎动脉先天性纤细、单干椎动脉形成基底动脉、一侧椎动脉缺如、椎动脉窗式变异、吻合变异进行统计。结果 544例颅内MRA图像均清晰显示了颅内血管,共189例椎动脉先天变异,占34.74%。其中男性92例,女性97例;年龄14~87岁。右侧椎动脉先天性纤细者106例,左侧椎动脉先天性纤细者39例,左侧椎动脉单干形成基底动脉者8例,右侧椎动脉单干形成基底动脉者7例,右侧椎动脉缺如者11例,左侧椎动脉缺如7例,右侧椎动脉窗式变异1例。结论 MRA能够准确地诊断椎动脉发育变异情况,并能为后循环疾病的诊断和治疗提供依据。Objective To investigate the diagnostic value of intracranial vertebral artery developmental variation of magnetic resonance angiography(MRA). Methods A total of 544 patients with ischemic stroke or transient cerebral ischemia by MRA were enrolled, which included 276 males and 268 females, aged 14-89 years old with mean age of 51.86 years old. The post-processing was performed at Leonardo workstation, then analyzed brain MRA examination images and observed vertebral artery developmental variation. The data were counted in arterial slender, single vertebral artery formation of basilar artery, one side vertebral artery absence, vertebral arterial fenestration and anastomotic variation. Results A total of 544 cases of intracranial MRA images clearly showed intracranial blood vessels. The 189 cases(34.74 %) were vertebral artery congenital variation, which included 92 males and 97 females, aged 14-87 years old. The 106 was arterial slender in the right vertebral artery and 39 in the left vertebral artery; vertebral artery formation of basilar artery of 8 in left and 7 in right, vertebral artery absence of 11 in right and 7 in left, and 1 was fenestration in right vertebral artery. Conclusion It is demonstrated that MRA could accurately diagnose the variability of vertebral artery development, and provide the basis for the diagnosis and treatment of post-circulatory diseases.
