摘 要:目的 探讨扩散张量成像(DTI)评价脂质体携载前列腺素E1(Lipo-PGE1)干预前后兔肝热缺血-再灌注损伤(WIRI)的价值。方法 健康成年新西兰大白兔50只,建立不同热缺血时间肝WIRI模型、干预组及假手术(sham)组模型。分组记为:S0(sham)组、S1(热缺血30 min)组、S2(热缺血40 min)组、S3(30 min干预)组、S4(40 min干预)组,每组10只。所有实验兔行常规MRI及DTI扫描,测量其表观扩散系数(ADC)、各向异性分数(FA)值。MR检查后取血清检测丙氨酸氨基转移酶(ALT)、天冬氨酸转氨酶(AST)及乳酸脱氢酸(LDH),处死后获取兔肝病理切片。采用组内相关系数(ICC)检验观察者间测量ADC、FA值一致性,应用单因素方差分析比较组间ADC、FA值差异,应用Spearman相关分析评价ADC值与临床化验指标的相关性。结果 2名观察者所测ADC、FA值的ICC值分别为0.850、0.764(ICC>0.75),一致性良好。随着热缺血时间的延长,ADC值呈下降趋势。sham组与热缺血组(S1、S2)组间ADC值差异有统计学意义(P均<0.05);干预组与对应热缺血组比较ADC值升高,组间差异均有统计学意义(P<0.05);sham组与干预组(S3、S4)组间ADC值差异均有统计学意义(P<0.05)。各组间FA值差异均无统计学意义(P>0.05)。ADC值与ALT、AST及LDH之间均呈显著负相关(r均>0.5 ,P均<0.05 )。结论 DTI能无创、定量评价肝WIRI及Lipo-PGE1对其的干预作用,对动态监测及评价肝WIRI具有重要临床意义。Objective To investigate the value of interventional effects of liposomal prostaglandin E1 (Lipo-PGE1) on the rabbit liver warm ischemia-reperfusion injury (WIRI) using diffusion tensor imaging (DTI).Methods 50 healthy adult New Zealand rabbits were randomly divided into five groups: SO : sham group, S1 : 30 min warm ischemic group,S2 : 40 min warm ischemic group, S3 : 30 rain interventional group,S4:40 min intervention group. S1 and $2 were groups of different warm ischemic time,S3 and S4 were interventional groups. Each group had 10 rabbits.All of them were examined by routine MRI and DTI,and their apparent diffusion coefficient (ADC) and fractionalanisotropy (FA) were measured. After MR examination, blood serum alanine aminotransferase (ALT),aspartate aminotransferase (AST) and lactate dehydrogenase {LDH) of all the rabbits were tested, we got liver pathology of each rabbit. Intra-class correlation coefficient (ICC)was used to analyse the consistency of ADC and FA values. One way analysis of variance (ANOVA) was performed to analyse the difference of ADC and FA values among groups. The correlation between ADC value and clinical indicators were evaluated using Spearman correlation analysis. Results ICC values of ADC (0. 850) and FA (0. 764) measurement showed moderate reproducibility.With the increased of warm ischemia time,ADC values decreased. There was signifi- cant difference in ADC values between the sham group and the warm ischemia group (S1, S2) (P〈0.05), and the ADC values ofthe intervention group was significantly higher than the warm ischemia group (P 〈 0.05). There was significant difference in ADC values between sham group and intervention group (S3, S4 ) (P〈0.05).No significant difference of FA was observed among groups (P〉0.05).ADC values were negatively correlated with ALT, AST and LDH (r〉0.5,P〈0.05).Conclusion DTI can noninvasively and quantitatively evaluate the interventional effect of Lipo-PGE1 o
