摘　　要：目的分析布鲁杆菌病中枢神经系统感染特征,探讨其临床诊断价值。方法选择2014年12月至2016年12月黑龙江省农垦总局总医院住院治疗的急性布鲁杆菌病并发中枢神经损害50例患者作为观察对象。临床诊断:并发脑膜炎30例(60.00%)、脑膜脑炎11例(22.00%)、脊髓炎7例(14.00%),脑脓肿2例(4.00%)。调查其住院病例资料,观察临床表现和体格检查及脑脊液(CSF)常规检查各项指标,分析感染临床特点。结果发热50例(100.00%),头痛40例(80.00%),恶心30例(60.00%),意识障碍2(4.00%),四肢感觉异常7例(14.00%),脑膜刺激征40例(80.00%),癫痫5例(10.00%),脑脊液检查颅内压增高30例(60.00%),白细胞计数增多45例(90.00%)。结论布鲁杆菌病诊断明确者,出现头痛,查体脑膜刺激征阳性,脑脊液白细胞增多,可以初步诊断为布鲁杆菌颅内感染。Objective to analyze characteristics of central nervous system infection of brucellosis and explore its clinical diagnostic value. Methods choose 50 cases acute brucellosis infection and central nerve injury patients hospitalized in Heilongjiang Reclamation Administration General Bureau General Hospital from December 2014 to December 2016 as observation objects. Clinical diagnosis： 30 cases of meningitis（60.00%）, 11 cases of meningoencephalitis（22.00%）, 7 cases of myelitis（14.00%）, 2 cases of brain abscess（4.00%）. Investigate medical record and data, observe indexes of clinical manifestations, physical examination and routine examination of cerebrospinal fluid（CSF）, and analyze clinical infection characteristics. Results there were 50 cases of fever（100.00%）, 40 cases of headache（80.00%）, 30 cases of nausea（60.00%）, 2 cases of consciousness disorder（4.00%）, 7 cases of extremities abnormality（14.00%）, 40 cases of meningeal irritation（80.00%）, 5 cases of epilepsy（10.00%）, 30 cases of intracranial hypertension of cerebrospinal fluid examination（60.00%）, 45 cases of leucocyte count increase（90.00%）. Conclusion with definite diagnosis, brucellosis infected patients have symptoms of headache, positive meningeal stimulation, leukocyte increase in cerebrospinal fluid, which can be initially diagnosed as brucellus intracranial infection.
