米力农治疗新生儿持续肺动脉高压的临床研究

摘　　要：目的探讨米力农治疗新生儿持续肺动脉高压(PPHN)的临床疗效。方法 30例新生儿持续肺动脉高压患儿,随机分为对照组和治疗组,各15例。对照组给予常规对症治疗,治疗组在常规对症治疗的基础上加用米力农治疗。比较两组患儿治疗前后左心室射血分数(LVEF)、动脉血氧饱和度(SaO_2)、肺动脉收缩压(SPAP)、心率及治疗效果。结果治疗前,两组患儿的LVEF、SaO_2、SPAP、心率比较,差异均无统计学意义(P>0.05);治疗后,两组患儿的LVEF、SaO_2、SPAP、心率均较本组治疗前改善,且治疗组改善程度优于对照组,差异均具有统计学意义(P<0.05)。治疗组总有效率为93.33%,高于对照组60.00%,差异具有统计学意义(P<0.05)。结论米力农对新生儿持续肺动脉高压有明显治疗效果,值得临床推广应用。Objective To investigate the clinical effect of Milrinone in the treatment of neonatal pulmonary hypertension. Methods A total of 30 neonates with persistent pulmonary hypertension were randomly divided into control group and treatment group, 15 cases in each group. The control group was treated with conventional symptomatic therapy, and the treatment group was treated with Milrinone on the basis of conventional symptomatic therapy. The left ventricular ejection fraction（LVEF）, arterial oxygen saturation（SaO2）, systolic pulmonary artery pressure（SPAP）, heart rate before and after treatment, the therapeutic effect between the two groups were compared. Results Before treatment, there were no significant differences in the LVEF, SaO2, SPAP and heart rate between the two groups（P〉0.05）. Compared with before treatmnet, the LVEF, SaO2, SPAP and heart rate of the two groups were improved after treatment, and the improvement degree of the treatment group were significantly better than those of the control group, the differences were statistically significant（P〈0.05）. The total effective rate of the treatment group was 93.33%, which was significantly higher than 60.00% of the control group, the difference was statistically significant（P〈0.05）. Conclusion Milrinone has obvious therapeutic effect on neonatal persistent pulmonary hypertension, and is worthy of clinical promotion and application.
