住院医嘱抗菌药物点评结果的相关研究

摘　　要：目的探讨住院医嘱抗菌药物的用药合理性。方法随机抽取200份住院医嘱抗菌药物病历进行点评,对其用药合理性进行分析,归纳、分析及总结不合理用药问题。结果 200份住院医嘱抗菌药物点评结果中,共有191份住院医嘱为用药合理医嘱,占总点评医嘱的95.50%(191/200);共有9份住院医嘱为用药不合理医嘱,占总点评医嘱的4.50%(9/200)。9份用药不合理医嘱包括抗菌药物分级管理落实不合理3份(占33.33%, 3/9),抗菌药物病程记录问题不合理2份(占22.22%, 2/9),用药频次不适宜1份(占11.11%, 1/9),围术期预防用抗菌药物不规范1份(占11.11%, 1/9),用法用量不适宜1份(占11.11%, 1/9),联合用药不适宜1份(占11.11%, 1/9)。结论结合住院医嘱抗菌药物点评存在的不合理用药现象,制定相关点评反馈机制,并落实执行,有利于提高用药合理性。Objective To explore the rational use of antibiotics drug in inpatient orders. Methods A total of 200 medical records of antimicrobial drugs were randomly selected for comment. The rationality of antimicrobial drug use was analyzed, and the problems of irrational drug use were induced, analyzed and summarized. Results In the 200 review results of antimicrobial drug orders, 191 copies of hospital orders were prescribed for rational use of drugs, accounting for 95.50%（191/200） of the total review. A total of 9 copies of hospital orders were unreasonable medical orders, accounting for 4.50%（9/200） of the total medical advice. 9 copies drug use unreasonable orders including antimicrobial drugs classification management to carry out the unreasonable in 3 copies（33.33%, 3/9）, 2 copies（22.22%, 2/9） issue record unreasonable course of antimicrobial drugs, drug use frequency of one copy（11.11%, 1/9） was not appropriate, perioperative antimicrobial prophylaxis was not standardized in one copy（11.11%, 1/9）. Usage and dosage was not suitable for one copy（11.11%, 1/9）, combination medication was not appropriate for one copy（11.11%, 1/9）. Conclusion Combined with the irrational use of antimicrobial drugs on the comments in inpatient orders, the review and feedback mechanism should be developed and implemented to improve the rationality of the use of antibiotics.
