参芎葡萄糖注射液对新生儿缺氧缺血性脑病的临床疗效观察及血清代谢组学变化

摘　　要：目的探讨参芎葡萄糖注射液对新生儿缺氧缺血性脑病(HIE)的临床疗效观察及其血清代谢组学变化,为临床治疗提供参考依据。方法选取2015年1月-2017年4月在该院治疗的HIE患儿67例,随机分为常规组(30例)和观察组(37例)。两组均给予支持和对症等综合治疗,观察组在上述治疗的基础上另给予参芎葡萄糖注射液静脉注射,疗程均为14 d。疗程结束后评价两组临床疗效,采用超高效液相色谱仪应用成分分析法分析两组患者治疗前后血清代谢差异物,并用EZinfo 2.0软件进行PCA分析潜在差异生物标记物。结果治疗14 d后,观察组与常规组临床总有效率分别为83.78%和73.33%,差异有统计学意义(P<0.01);观察组患儿的意识障碍恢复时间、原始反射恢复时间和肌张力恢复时间明显缩短(P<0.01);观察组患儿治疗后血清血胆碱、乳酸、谷氨酸含量低于常规组治疗后,肌酸、牛磺酸、苏氨酸、N-乙酰天门冬氨酸含量升高,差异有统计学意义(P<0.01)。结论参芎葡萄糖注射液治疗HIE具有较好的临床疗效,可明显改善HIE症状、体征,同时血清中胆碱、乳酸、肌酸、牛磺酸、苏氨酸、谷氨酸、N-乙酰天门冬氨酸含量出现变化。Objective To explore the clinical effect of Shenxiong glucose injection in treatment of neonatal hypoxic-ischemic encephalopathy（ HIE） and changes of serum metabonomics,provide a reference basis for clinical treatment of neonatal HIE. Methods From January 2015 to April 2017,67 neonates with HIE treated in Ezhou Central Hospital were selected and randomly divided into conventional group（ 30 neonates） and observation group（ 37 neonates）. The neonates in the two groups were treated by comprehensive treatment（ support and symptomatic therapy）. The neonates in observation group were treated by intravenous injection of Shenxiong glucose injection based on comprehensive treatment. The neonates in the two groups were treated for 14 days. After treatment,the clinical effects in the two groups were evaluated. Componential analysis was used to analyze serum metabolism difference before and after treatment in the two groups using superhigh performance liquid chromatography. PCA was used to analyze the potential difference biomarkers using EZinfo 2. 0 software.Results After 14 days of treatment,the total effective rates in observation group and conventional treatment group were 83. 78% and 73. 33%,respectively,there was statistically significant difference between the two groups（ P〈0. 01）. The recovery time of consciousness disturbance,primary reflex,and muscle tension in observation group were statistically significantly shorter than those in conventional treatment group（ P〈0. 01）. After treatment,the levels of serum choline,lactic acid,and glutamic acid in observation group were statistically significantly lower than those in conventional treatment group（ P〈0. 01）,the levels of creatine,taurine,threonine,and N-acetylaspartate in observation group were statistically significantly higher than those in conventional treatment group（ P 〈0. 01）.
