摘　　要：目的:研究miR-23b-3p在皮肌炎(DM)患者血清中的表达水平,探讨其在DM中的临床意义。方法:收集2011年3月~2016年2月我院风湿免疫科住院的DM患者(活动期)血清49例,健康对照血清30例,提取血清总RNA,采用实时荧光定量PCR方法检测血清中miR-23b-3p相对表达量,分析与临床特征的相关性。结果:DM患者血清miR-23b-3p相对表达量(1.98±0.86)显著低于健康对照组(3.02±1.13,P<0.01),DM合并"技工手"患者血清miR-23b-3p表达水平显著低于未合并"技工手"患者(P<0.05)。相关性分析显示,miR-23b-3p与肌酸激酶(CK)呈负相关(r=-0.286,P<0.05),与乳酸脱氢酶(LDH)、谷草转氨酶(AST)、谷丙转氨酶(ALT)、C反应蛋白(CRP)、血沉(ESR)等实验室指标无相关性。结论:DM患者中miR-23b-3p表达下调可能通过多个环节参与DM的发病过程。Objective：To observe the levels of miR-23b-3p in serum of patients with dermatomyositis（DM）and to explore its clinical significance in DM.Methods：Forty-nine patients with DM in active period between March 2011 and February 2016 in our hospital were enrolled in this study.Thirty healthy adults were selected as healthy controls. Serum samples were collected from these patients,and total RNAs were extracted from these samples.The expressions of miRNA-23b-3p were detected using fluorescent quantitation real-time quantitative polymerase chain reaction（RT-PCR）technique.Then the correlations between different clinical features were analyzed.The measurement data were compared between the two groups using the χ~2 test.The comparison of the count data was performed by Mann-Whitney U test and the relevance was analyzed using Spearman correlation analysis.Results：The relative expressions of serum miR-23b-3p in DM patients（1.98±0.86）was significantly lower than that in healthy controls（3.02±1.13;P〈0.01）.Serum miR-23b-3p levels in DM patients with mechanic＇s hands were significantly lower than those without（P〈0.05）.Correlation analysis showed that miR-23b-3p was negatively correlated with creatine kinase（r =-0.286,P〈0.05）.Conclusion：The down-regulation of miR-23b-3p expression in DM patients may participate in the pathogenesis of DM through multiple aspects.
