摘  要:目的评估使用人工听觉装置耳聋儿童的听觉能力,探讨患儿听觉能力与言语理解能力的相关性,为临床工作中常用听觉能力评估问卷的选择提供参考。方法共有27例来自淄博地区的感音神经性语前聋儿童参与本次研究。男14例,女13例。耳聋程度为中度至极重度。生理年龄9~95个月,平均(42±25)个月。干预年龄1~72个月,平均(26±24)个月。听力年龄1~60个月,平均(16±14)个月。均为双耳助听。患儿听觉能力使用婴幼儿有意义听觉整合量表(infant-toddler meaningful auditory integration scale,IT-MAIS)/有意义听觉整合量表(meaningfulauditor y integrationscale,MAIS)量表、听觉行为分级(categories ofauditoryperformance,CAP)量表、父母评估孩子听说能力表现(parents evaluation of aural/oral performance of children,PEACH)量表进行评估,并对评估结果进行线性相关与回归分析。结果患儿IT-MAIS/MAIS、CAP和PEACH得分分别为25.67±11.94/29.80±8.44、4.52±1.93和32.52±12.40,得分均呈正态分布(P=0.936、0.699、0.307)。IT-MAIS/MAIS得分与PEACH得分呈显著相关(r=0.947,P=0.000);CAP得分与PEACH得分呈显著相关(r=0.893,P=0.000);IT-MAIS/MAIS得分与CAP得分呈显著相关(r=0.846,P=0.000)。结论评估患儿听觉能力的各个问卷之间具有显著相关性。IT-MAIS/MAIS对问卷人的专业水平要求较高并且用时较长;CAP问卷相对简单实用,评估所需时间短;PEACH问卷可用于评估各个年龄阶段耳聋儿童在日常生活中听觉能力,适用范围较广。鉴于各个问卷具有显著相关又各有适用特点,因此可根据临床科研实际、专业人员情况、患儿情况、家长配合度选择使用。OBJECTIVE To evaluate the auditory ability in children with artificial hearing devices, explore the relationship between the auditory ability and speech intelligibility, and provide reference for the choice of the common assessment questionnaires in clinical work. METHODS A total of 27 children with preling ual sensorineural hearing loss from Zibo participated in this study, 14 were boys and 13 were girls. Their hearing level was from moderate to profound. Their chronological age at evaluation ranged from 9 to 95 months with a mean of 42 months. Their chronological age at intervention ranged from 1 to 72 months with a mean of 26 months. Their hearing age at evaluation ranged from 1 to 60 months with a mean of 16 months. They were all with bilateral aided hearing. IT-MAIS/MAIS, CAP and PEACH were used to evaluate their auditory ability. A linear and regression analysis was done in the statistic procedure. RESULTS The means and standard deviations of scores of IT-MAIS/MAIS, CAP and PEACH were 25.67±11.94/29.80±8.44, 4.4±1.9, 2.8±1.4 respectively. The scores of IT-MAIS/MAIS, CAP and PEACH in this group of children showed a normal distribution respectively(P =0.936, 0.699, 0.307). Scores of IT-MAIS/MAIS and PEACH were significantlycorrelated(r =0.947, P =0.000). Scores of CAP and PEACH were also significantly related(r =0.893, P =0.000). Scores of IT-M A IS/M A IS and CA P were also sig nif icantly related(r =0.846, P =0.000). CONCLUSION There were signif icant relationships among the questionnaires of auditory ability assessment in children with bilateral aided hearing. More professional skills are needed for IT-MAIS/MAIS, CAP is simpler and easily understood, PEACHis not limited by the ages, but more detailed than other questionnaires. Therefore, the application of questionnaires could be chosen according to clinical practice, skilled professionals, situation of children and the cooperation of parents.
