贝克抑郁量表、贝克焦虑量表评价下牙列缺损患者负性情绪对义齿种植后疼痛的影响

摘　　要：目的：分析BDI与BAI评价下牙列缺损患者术前负性情绪对义齿种植之后疼痛感受的影响。方法：我院从2013年4月至2016年5月收治的261例下牙列缺损患者作为研究对象,所有患者均行口腔种植术治疗,术前应用贝克抑郁量表（Beck Depression Inventor,BDI）与贝克焦虑量表（Beck Anxiety Inventory,BAI）对患者进行心理评价,将其分为负性情绪组与正常组,术后应用视觉模拟评分法（visual analogue scale,VAS）、现时疼痛强度（present pain intensity,PPI）以及疼痛分级指数（pain rating index,PRI）,分析患者负性情绪对种植后疼痛感受的影响。结果：术前负性情绪组与正常组患者BDI（t=55.516）与BAI（t=68.888）评分差异较大,数据检验结果有统计学意义（P〈0.05）;术后两组患者在VAS（t=13.204）、PPI（t=24.049）以及PRI（t=27.557）评分中均存在较大差异（P〈0.05）;相关性分析结果显示,术前患者负性情绪与种植后疼痛呈正相关（P〈0.05）,但两组患者红、白美学评分差异不明显,无统计学意义（P〉0.05）。结论：牙列缺损患者义齿种植治疗,术前患者的负性情绪会直接影响术后的疼痛感受,负性情绪程度越深其疼痛感受越大。Objective：To study the effect of preoperative negative emotions evaluated by BDI and BAI scales on pain after denture implantation in patients with dentition defect.Methods：A total of 261 patients with inferior dentition defect admitted to our hospital from April 2013 to May 2016 were enrolled in this study.All patients underwent oral implant surgery.The Beck Depression Inventor（BDI））and the Beck Anxiety Inventory（BAI）were used to evaluate the emotion of patients and all patients were divided into two groups：Negative emotion group and normal group.Postoperative visual analogue scale（VAS）,present pain intensity（PPI）and pain rating index（PRI）were used to analyze the effect of negative emotions on pain perception after implantation.Results：There was significant differences between BDI and BAI in preoperative negative emotional group and normal group（t=55.516,68.888,P〈0.05）.There were significant differences in VAS,PPI and PRI scores between the two groups after operation（t=13.204,24.049,27.557;P〈0.05）.The correlation analysis showed that the negative emotions in preoperative patients were positively correlated with postoperative pain（P〈0.05）,but there was no significant difference between the two groups in the red and white aesthetic scores（P〈0.05）.Conclusion：For dental prosthesis implantation in patients with dentition defect,the negative emotions before surgery patients will have a direct impact on postoperative pain perception,and the greater the degree of negative emotion is,the more the pain is.
