平衡肌力法松解内括约肌治疗重度肛裂的疗效观察

摘　　要：为观察平衡肌力法松解内括约肌治疗重度肛裂的临床疗效,将120例重度肛裂患者随机分为观察组和对照组,各60例,观察组采用平衡肌力法松解内括约肌治疗,取2个对称位置切开松解内括约肌(即膀胱截石位3、9点位或5、7点位对称松解肛门内括约肌);对照组采用常规后正中位内括约肌切断术治疗。对比观察2组患者术后肛门疼痛、肛门功能(肛门失禁)、创面愈合时间、治愈率及半年后复发率。结果显示,2组治愈率比较差异无统计学意义,P>0.05;但在创面愈合时间、术后肛门疼痛情况、肛门失禁情况及术后半年复发情况方面,观察组明显优于对照组,P <0.05。结果表明,平衡肌力法松解内括约肌治疗重度肛裂能有效缩短创面愈合时间,减轻肛门疼痛,减少肛门功能损伤,降低术后复发率,保证肛门外观正常,值得临床推广。To observe the clinical effect of severe anal fissure treated with muscle-strength balanced meth- od for lysis internal-sphincter, 120 cases were randomly divided into observation group and control one（ n =60,for each）, the former received above muscle-strength balanced method for lysis internal-sphincter through both symmetrical site incisions（bladder lithotomy position 3,9 point or 5,7 points）,the latter re- ceived routine incision of internal-sphincter at median position, both groups′ postoperative anal pain and function（incontinence） ,as well as wound-surface healing time, curative rate and recurrence rate within 0.5 years were compared.As results,in curative rate there was no statisticai difference between the two groups （ P 〉0.05） ,but in above rest indexes observation group was all superior to control one（ P 〈0.05）.Results show that muscle-strength balanced method can effectively shorten wound-surface healing time, reduce pain of anus,reduce function truama of anus, decrease postoperative recurrence, and keep intact appearance of anus.It is worthy of clinical popularization and application.
