摘 要:目的:探讨高原环境下大鼠力竭运动后急性肾损伤(acute kidney injury,AKI)及其新标志物的灵敏度与可靠性。方法:将72只雄性Wistar大鼠随机分为亚高原组和高原组,每组36只。再按照取样时间不同分别将亚高原组和高原组随机分为6个亚组,每组6只。通过对大鼠进行负重游泳至力竭,记录游泳时间,检测力竭后不同时间血清中肾损伤分子-1(kidney injury molecule-1,KIM-1),白介素-18(interleukin-18,IL-18)及中性粒细胞明胶酶相关载脂蛋白(neutrophil gelatinase-associated lipocalin,NGAL)的含量。结果:亚高原组大鼠力竭运动后各时间点血清中KIM-1及IL-18无显著变化;NGAL含量在力竭12h后升高(P<0.05),48h时持续增高(P<0.01)。高原组KIM-1及NGAL的含量在力竭后12h时升高(P<0.05),24h时持续升高(P<0.01),48h时仍然维持高浓度;IL-18在力竭后6h即刻升高(P<0.01),之后一直保持高浓度且呈持续增高趋势(P<0.01)。相同时间点高原组与亚高原组各指标含量比较,NGAL变化最大,IL-18次之,KIM-1的变化时间滞后且在48h时有接近趋势。结论:与亚高原组相比,高原组大鼠力竭运动后产生较为严重的AKI反应。在预测AKI方面,NGAL最为明显,IL-18次之,KIM-1较弱。Objective:To investigate the sensitivity and reliability of the new biomarkers of the acute kidney injury (AKI) in rats after exhausting exercise at plateau.Methods: Seventy--two rats were randomly divided into plateau group and sub--plateau group,36 rats in each group. Then plateau group and sub- plateau group were randomly divided into six subgroups respectively,6 rats in each subgroup.The rats were carried out weight--bearing swimming test till exhaustion,then the swimming time was recorded and the serum kidney injury molecule 1 (KIM--1),interleukin--18(IL--18) and neutrophil gelatinase associated lipocalin(NGAL) were determined.Results:At sub--plateau,there were no obvious changes of serum KIM--1 and IL--18 in each time point after exhausting exercise.The content of serum NGAL rose after 12 h of exhaustive,and continued to rise at 48 h(P〈0.01). At plateau,the content of serum NGAL and K1M-1 significantly inceased at 12 h (P〈0.01),there were still inreasing when it was up to 24 h (P〈0.01),and remained high at 48 h. The content of IL-18 rose immediately after exhaustion(P〈0.01),and kept high all the time (P〈0.01).The contents of the KIM--1,IL-18 and NGAL between plateau group and sub--plateau group at the same time points showed that serum NGAL had the most obvious changes,IL-18 took second place,the change time of KIM-1 laged and had the same trend at 48 h. Conclusion: Compare with the sub--plateau group,rats in the plateau group had more severe AKI after exhausting exercise.Serum NGAL, KIM-1 and IL-18 can be markers of AKI, but NGAL shows much more sensitivity, reliability and stablity.
