器械相关感染目标性监测运行模式的建立及再评价研究

摘　　要：目的探讨器械相关感染目标性监测运行模式对控制器械相关感染的意义。方法建立器械相关感染目标性监测运行模式,调查某医院2013年1月-2016年12月重症监护室、神经内科、神经外科、老年病科使用呼吸机、中央导管和尿管的住院患者。结果 2013-2016年重症监护室、神经内科、神经外科和老年病科住院患者医院感染发病率和例次发病率均降低;医院感染日发病率和日例次发病率也均呈下降趋势;呼吸机使用率、尿管使用率、中央导管使用率比较,差异均具有统计学意义。导尿管相关尿路感染发病率、中央导管相关血流感染发病率呈下降趋势(χ2=7.966、7.857,P<0.05)。结论通过建立器械相关感染目标性监测运行模式开展目标性监测可有效控制导尿管相关尿路感染和中央导管相关血流感染发病率,从而减少医院感染的发生,对临床工作有一定的积极意义。Objective To investigate the significance of device-related infection target monitoring mode of operation in the control of device-related infection. Methods A device-related infection target monitoring mode of operation was established to investigate inpatients with ventilators,central catheters and urinary catheters in the intensive care unit,neurology,neurosurgery,and geriatrics from January 2013 to December 2016. Results The incidence of nosocomial infection and the case-time infection rate were significantly decreased in the intensive care unit,neurology department,neurosurgery and geriatrics hospitalized from 2013 to 2016. The daily infection rate and the case-time daily infection rate were also declining. The utilization rates of ventilators,urinary catheters and central catheters were statistically significant. The incidence of catheter-associated urinary tract infection（CAUTI） and the incidence of central catheter-related bloodstream infection（CLABSI） showed a decreasing trend,the differences were statistically significant（χ2= 7. 966,7. 857,P〈 0. 05）.Conclusion Through the establishment of device-related infection target monitoring mode of operation to carry out target monitoring,we can effectively control CAUTI and CLABSI,reduce the incidence of nosocomial infection,which has a certain positive significance to clinical work.
