儿童医院住院患者感染病原菌分布及其耐药性研究

摘　　要：目的了解儿童医院住院患者临床分离病原菌的分布及其耐药性,为医院感染预防控制提供循证依据。方法采用回顾性调查方法,对某儿童医院住院患者送检的病原学标本检测结果进行调查与分析。结果共收集146 394份病原学标本,检出病原菌51 008株,阳性率为37.84%。检出的病原菌中,革兰阴性菌占61.62%,革兰阳性菌占34.60%,真菌占3.78%。大肠埃希菌及肺炎克雷伯菌中产超广谱β-内酰胺酶菌株的检出率分别为57.41%和46.03%。耐甲氧西林金黄色葡萄球菌和凝固酶阴性葡萄球菌检出率呈现逐年上升趋势。白色假丝酵母菌和近平滑假丝酵母菌对抗菌药物仍较敏感性。结论儿童医院临床分离的病原菌耐药率呈现逐渐增长趋势,应根据药敏试验结果合理使用抗菌药物。Objective To realize the distribution and drug resistance of pathogenic bacteria isolated from patients in children＇s hospital and provide evidence for the prevention and control of hospital infection. Methods Retrospective investigation was used to review and analyze the test results of pathogenic specimens sent for examination from patients in a children＇s hospital. Results A total of 146 394 pathogenic specimens were collected and 51 008 strains were detected out,the positive rate was 37. 84%. Among those detected,Gram-positive bacteria accounted for 34. 60%,Gram-negative bacteria accounted for 61. 62% and fungus accounted for 3. 78%. The detection rate of ESBLs among Escherichia coli and Klebsiella pneumoniae were 57. 41% and 46. 03%. The resistance rate of Methoxicillin in Coagulase-negative Staphylococcus aureus and Staphylococcus aureus appeared an increasing tendency. Candida albicans and Candida parapsilosis were still sensitive to antibiotics. Conclusion The drug resistance rate appears an increasing tendency. It is suggested that antibiotics should be used according to the results of susceptibility test rationally.
