团体心理治疗在脑卒中失语症患者康复治疗中的应用

摘　　要：目的：探讨团体心理治疗在脑卒中失语患者康复治疗中的应用。方法：将脑卒中失语症68例患者随机分为普通治疗组和心理治疗组,每组34人。普通治疗组给予语言康复Schuell刺激法,每天进行1次,每次30分钟;心理治疗组患者给予语言康复治疗的同时,在住院期间和出院后连续参加每周2次、共4周的团体心理治疗。治疗前后采用西方失语成套（WAB）测试、波士顿诊断性失语（BDAE）严重程度分级标准进行失语症评价和分级,采用卒中后失语患者抑郁问卷（医院版）（SADQ-H）评分评估失语症患者的抑郁状态。结果：4周后心理治疗组抑郁评分改善较普通治疗组明显（t=2.421,P=0.0183）,心理治疗组WAB的失语商（AQ值）改善较普通治疗组明显（t=-2.485,P=0.0155）;治疗后两组波士顿失语严重程度分级比较,心理治疗组改善也较普通治疗组明显（Z=-2.742,P=0.036）。结论：团体心理治疗配合言语康复治疗既能改善脑卒中失语症患者的抑郁情绪,又能更好地改善患者语言功能的康复。Objective：To explore the application of group psychotherapy in rehabilitation treatment of stroke aphasia patients.Methods：A total of 68 stroke patients with aphasia were randomly divided into the general treatment group and the psychotherapy group,34 in each group. The general treatment group received language rehabilitation schuell stimulation method,1 times a day,30 minutes each time;The patients in the psychotherapy group were given language rehabilitation therapy,and they participated in the group psychotherapy 2 times a week and 4 weeks during hospitalization and after discharge.Aphasia was assessed and graded by Western aphasia battery（ WAB） and Boston dignostic aphasia examination（BDAE） before and after treatment.Stroke aphasia depression questionnaire hospital version（SADQ-H）score was used to assess the depressive status of aphasic patients.Results：After 4 weeks,the scores of depression in the psychotherapy group were significantly better than that in the ordinary treatment group（ t = 2.421,P〈0.05）,the improvement of aphasia quotient（AQ value）of WAB in psychotherapy group was more obvious than that in normal treatment group（t=-2.485,P〈0.05）;After treatment,the severity of aphasia in the two groups of Boston was compared,and the improvement of the psychological treatment group was also more obvious than that of the ordinary treatment group（ Z =-2. 742,P〈0. 05）. Conclusion：Group psychotherapy combined with speech rehabilitation therapy can not only improve the depression of stroke patients with aphasia,but also ultimately improve the rehabilitation of patients＇ language function.
