庆大霉素与多西环素联合治疗对急性布氏杆菌病患者血清指标的影响及疗效评价

摘　　要：目的探讨庆大霉素和多西环素联合治疗布氏杆菌病患者血清指标的变化并评价疗效。方法回顾性分析辽源市第二人民医院感染科于2014年1月至2017年12月收治的布氏杆菌病患者100例资料,按照入院治疗时间分为观察组和对照组,每组50例。对照组患者在常规对症治疗基础上口服利福平+多西环素片治疗,观察组患者在常规对症治疗基础上口服庆大霉素+多西环素治疗。比较两组患者的临床疗效、降钙素原(PCT)与C反应蛋白(CRP)水平及不良反应发生情况。结果观察组患者总有效率为96.0%,略高于对照组的92.0%,但差异无统计学意义(P>0.05)。两组患者治疗前PCT、CRP表达差异无统计学意义(P>0.05),治疗后上述指标均显著降低,差异有统计学意义(P<0.05)。观察组患者治疗后PCT、CRP水平低于对照组,差异有统计学意义(P<0.05),观察组患者不良反应发生率为12.0%,稍高于对照组的8.0%,但差异无统计学意义(P>0.05)。结论庆大霉素与多西环素联合治疗布氏杆菌病疗效显著,不良反应少,具有临床诊疗价值。Objective Investigate the changes of serum indexes in brucellosis patients treated with gentamicin and doxycycline and evaluate their therapeutic effects.Methods A retrospective analysis of 100 cases of brucellosis in Infectious Disease Department;Second people＇s Hospital of Liaoyuan in December 2017~January 2014, was divided into the observation group and the control group according to the time of admission, 50 cases in each group.The control group was treated with rifampin ＋ doxycycline tablets on the basis of routine symptomatic treatment, and the observation group was treated with gentamicin ＋ doxycycline on the basis of routine treatment. The efficacy, PCT and CRP levels,and adverse reactions were compared between the 2 groups.Results The total effective rate of the observation group 96.0%（48/50） was higher than that of the control group 92.0%（46/50））,（P〉0.05）. There was no significant difference in the expression of PCT and CRP between the 2 groups before treatment（P〈0.05）, and the above indexes were significantly decreased after treatment（P〈0.05）. The level of PCT and CRP in the observation group was lower than that of the control group. The difference was statistically significant（P〈0.05）, the incidence of adverse reaction in the observation group was 12%（6/50） slightly higher than that of the control group 8%（4/50））, the difference was not statistically significant（P〈0.05）.Conclusions Gentamicin combined with doxycycline is effective in the treatment of brucellosis with little side effects and clinical value.
