层级护理模式对血液透析患者负性情绪和血透并发症的影响

摘　　要：目的：探讨层级护理模式对血液透析患者负性情绪和血透并发症的影响。方法：选取2014-2017年我院收治的160例血液透析患者作为研究对象,采用回顾性分析法分析所有患者的临床资料,按照资料中其护理方法分为对照组和研究组各80例,对照组患者在所患疾病临床治疗上予以常规护理,研究组予以层级护理模式,两组患者均护理4周,护理结束后比较两组患者焦虑抑郁、血透并发症情况,并记录两组患者对护理的满意度情况。结果：（1）研究组护理后焦虑自评量表（SAS）、抑郁自评量表（SDS）评分均较对照组降低明显,差异具有统计学意义（t=-5.127,-6.313;P〈0.001）;（2）研究组患者并发症发生率（21.25%）明显低于对照组（40.00%）,差异具有统计学意义（χ~2=6.619,P〈0.05）;（3）研究组护理总满意度（95.00%）明显高于对照组（65.00%）,差异具有统计学意义（χ~2=22.500,P〈0.01）。结论：对血液透析患者实施层级护理模式管理可明显减轻其焦虑和抑郁等负性情绪程度,降低并发症发生率,且更易被患者所接受,可作为血液透析患者首选的护理模式。Objective： To explore the effect of stratified nursing model on negative emotion and hemodialysis complications in patients with hemodialysis.Methods： A total of 160 patients with hemodialysis from 2014 to 2017 were selected as the subjects,and the clinical data of all patients were analyzed by retrospective analysis.According to the data in their care methods they were divided into control group and study group,80 cases in each group.The control group was routinely treated in the clinical treatment of the disease,and the study group was given a hierarchical nursing model. All patients were treated for 4 weeks.At the end of the nursing group,the patients with anxiety,depression and hemodialysis were compared and the satisfaction of the two groups was recorded.Results：（1）The SAS and SDS scores of the study group were significantly lower than those of the control group（ t =-5.127,-6.313; P〈0.001）.The incidence of complications was significantly lower in the study group（ 21.25%） than that in the control group（ 52.50%）.The difference was statistically significant（ χ~2= 6.619,P〈0.05）.The overall satisfaction（ 95. 00%） was significantly higher in the study group than that in the control group（ 65.00%）.The difference was statistically significant（ χ~2= 22.500,P〈0.01）. Conclusion： The management of stratified nursing model in hemodialysis patients can significantly reduce the degree of negative emotion of anxiety and depression,reduce the incidence of complications,and be more easily accepted by patients,which can be used as the preferred nursing model for hemodialysis patients.
