大鼠胃切口缝合后局部注射脂肪间充质干细胞对伤口愈合的影响

摘　　要：目的探讨大鼠胃缝合处局部注射脂肪间充质干细胞(ADMSC)对伤口愈合的影响。方法将32只大鼠完全随机分成对照组和局部注射ADMSC组(ADMSC组),各16只。两组均行胃穿孔缝合模型,对照组不做处理,ADMSC组局部注射ADMSC;术后3及5 d观察胃穿孔愈合情况,HE染色和三色法观察术后组织恢复情况;同时应用一抗标记切片,观察胃穿孔缝合处IL-6阳性表达情况。结果与对照组相比,ADMSC组术后3及5 d切口处愈合更好,更少缝合口瘘发生,特别是术后5 d更显著;局部注射ADMSC组术后5 d更多肉芽和上皮增生,组织水肿显著减轻;同时,IL-6阳性标记区域占比在术后5 d显著降低,炎症反应降低。结论局部注射ADMSC可减轻大鼠胃穿孔缝合处炎症反应,可促进伤口的愈合。Objective To investigate the effect of local injection of adipose mesenchymal stem cell （ADMSC） at gastric suture on wound healing in rats. Methods Thirty-two rats were randomly divided into control group and local injection ADMSC group ,with 16 rats in each group. Both groups were sutured after gastric incision. Tthe control group received local injection of adipose mesenchymal stem cells without treatment, while local injection ADMSC group were treated with local injection of ADMSC. The healing of the incision was observed for 5 days after oper- ation, and the tissue recovery was observed by HE staining and trichromatic method. The positive expression of IL-6 was observed in the suture of gastric perforation. Results Compared with the ADMSC group, the incision healing was better and the suture fistula was less, especially on the 5th day after the operation; more granulation and epithelial hyperplasia were observed in the ADMSC group, and tissue edema was alleviated significantly in the local injection of ADMSC group on the 5th day after the operation. Especially on the 5th day after the operation, there were more granulation and epithelium hyperplasia in the local injection group. At the same time, the proportion oflL-6 positive area decreased significantly at 5 days after operation, and the inflammatory reaction decreased. Conclusions Local injection of ADMSC can attenuate the inflammatory reaction and promote wound healing at the suture of gastric perforation.
