摘  要:目的探讨昼夜节律失调女性血皮质醇和褪黑素节律的变化与胰岛素抵抗的关系。方法选择我院女性护士轮夜班的26例、白班12例,并根据焦虑自评量表(SAS)评分把轮夜班护士分为夜班不适应组(10例,SAS评分≧50)及夜班适应组(16例,SAS评分<50分),分别检测夜班组下夜班当日、休息日及白班组睡眠前及清醒时血皮质醇和褪黑素水平,下夜班当日空腹血糖及胰岛素水平。结果夜班不适应组下夜班当日睡眠皮质醇节律出现倒转,与夜班适应组及白班组明显不同;夜班不适应组褪黑素在睡眠前后之差明显小于夜班适应组及白班组;夜班不适应组的胰岛素抵抗水平明显高于其他两组,并具有统计学意义(P<0.05)。结论皮质醇及褪黑素节律变化对于认识夜班对女性护士健康的影响起着重要作用,有利于及早干预。Objective To explore the relationship between the rhythm change of blood cortisol and melatonin and insulin resistance in circadian disorder women. Methods Female nurses included 26 cases of night shift and 12 cases of day shift in our hospital were collected. According to the results of Self-Rating Anxiety Scale(SAS) of night shift,night shift nurses were divided into the non-adapted night shift group and adapted night shift group. The levels of blood cortisol and melatonin were measured at different times such as the night shift group at the same day and rest days and the day shift group at pre-sleep and awake. Results The cortisol rhythm was reversed in the non-adapted night shift group,which was different from the adapted night shift group and the day shift group. The difference of melatonin between before and after sleep was less in the non-adapted night shift group than the other two groups. Meantime,the level of the insulin resistance was higher in the non-adapted night shift group than the other two groups( P〈 0.05). Conclusion The rhythm change of cortisol and melatonin play an important role in female nurses health,especially in the non-adapted night shift nurses.
