Th17细胞相关因子在银屑病患者外周血清中的检测及其临床意义

摘　　要：目的探讨Th17细胞相关因子在银屑病患者发病中的作用和意义。方法利用Luminex 200液相芯片检测系统检测70例银屑病患者(银屑病组)和70例正常对照者(对照组)血清中IL-17A、IL-17F、IL-17E/IL25、白细胞介素6(IL-6)、肿瘤坏死因子α(TNF-α)表达水平,分析其在银屑病患者与正常对照组间,在银屑病不同病期、类型及严重程度间的差异。结果银屑病患者外周血清中TNF-α水平显著升高(P=0.000),不受银屑病病型、病期和皮损严重程度影响;IL-17A水平较正常升高(P<0.05),主要发生于寻常型银屑病患者、疾病进行期,并与银屑病皮损严重程度相关;特殊型银屑病患者血清IL-17E水平低于正常对照和寻常型银屑病患者,而IL-6水平高于正常人和寻常型,差异均有统计学意义(P<0.05);IL-17F水平仅在轻度银屑病组中显示高于正常(P=0.016)。结论 TH17细胞相关因子IL-17A、IL-17F、IL-17E/IL25及IL-6、TNF-α相互协同,共同参与了银屑病的发生发展,并为银屑病的生物靶向治疗开辟了新思路。ObjectiveTo explore the role and significance of Th17 cell related cytokines in the pathogenesis of psoriasis. MethodsThe levels of IL-17A, IL-17F, IL-17E/IL25, IL-6 and TNF-α in peripheral serum of 70 patients with psoriasis and 70 normal controls were detected by Luminex 200 liquid chip detecting system.The difference in the levels of those cytokins between patients with psoriatics at different stages, of different types and with different levels of severity and normal controls was analyzed. ResultsThe serum concentration of TNFα of patients with psoriasis was obviously higher than that of normal controls（P=0.000）. The level of IL-17A was elevated at the active stage of psoriasis, and was related to the severity of skin lesions.In special types of psoriasis, the level of IL-17E was lower than that of normal controls or ordinary patients（P〈0.05）, while the level of IL-6 was elevated, and the difference was of statistical significance（P〈0.05）. The level of IL-17F was higher only in patients with mild psoriasis（P=0.016）. ConclusionTh17 cell related cytokines, including IL-17A, IL-17F, IL-17E/IL25, IL-6 and TNF-α, were involved synergistically in the occurrence and development of psoriasis. This finding will contribute to the biotargeted treatment of psoriasis.
