重组人酸性成纤维细胞生长因子联合利福平油纱对深Ⅱ°烧伤肉芽创面的疗效分析

摘　　要：目的探讨重组人酸性成纤维细胞生长因子(rhaFGF)联合利福平油纱对深Ⅱ°烧伤肉芽创面的疗效。方法采用便利抽样法,选取2017年1-12月收入我科的74例四肢深度烧伤后期患者作为干预组,对1%总体表面积(TBSA)深Ⅱ°烧伤肉芽组织创面应用利福平油纱联合重组人酸性成纤维细胞生长因子治疗。另将同期转入的74例同类患者作为对照组,选取与干预组创面对应或相邻的部位,常规应用皮维碘软膏治疗。记录患者年龄、性别、烧伤总面积、深Ⅱ°烧伤面积、总住院时间,并比较烧伤肉芽组织创面细菌培养转阴率和不同时间创面愈合百分率。结果两组患者的年龄、性别、烧伤总面积及深Ⅱ°烧伤面积的差异均无统计学意义(P>0.05);与对照组比较,住院2周时干预组的细菌总转阴率显著升高(P<0.05),创面愈合率显著提升(P<0.05),总住院时间差异无统计学意义(P>0.05)。结论应用重组人酸性成纤维细胞生长因子联合利福平油纱治疗深Ⅱ°烧伤肉芽组织创面,能有效控制烧伤肉芽组织创面感染的菌群,并加速烧伤肉芽组织创面的愈合。Objective To investigate the effect of recombinant human acidic fibroblast growth factor（ rhaFGF） combined with rifampin oil gauze on deep second degree burn wounds. Methods Using the convenient sampling method,74 patients with deep burn of extremities and treated between January 2017 and December 2017 were selected as the intervention group. Rifampin combined with rhaFGF was used to treat the wounds of 1% TBSA deep second degree burn granulation tissue. Another 74 similar patients who were treated in the same period were selected as the control group. The age,sex,total area of burn,area of deep second degree burn wounds and hospital stay were recorded,and the rate of bacterial culture conversion and the percentage of wound healing at different time points were compared.Results There was no significant difference in the distribution of age,sex,total area of burn,area of deep second degree burn wounds or the total length of hospital stay between the two groups（ P〈0.05）.Compared with the control group,the total bacterial negative rate in the intervention group increased significantly（ P〈0.05）,so was the percentage of wound healing（ P〈0.05）.Conclusion The application of rhaFGF combined with rifampin oil gauze in the treatment of burn granulation tissue can effectively control the bacterial infection of burn granulation wound and accelerate the healing of burn granulation tissue.
