自制中药协定方治疗慢性荨麻疹疗效观察及对血清组胺、IL-2、IL-6水平的影响

摘　　要：目的探讨本院自制口服中药协定方对慢性荨麻疹的疗效及可能的作用机制。方法慢性荨麻疹患者随机分为西替利嗪组、协定方组和联合用药组,健康体检者为对照组。治疗后2周和4周对临床疗效进行评分和观察。用酶联免疫吸附法测定细胞因子白细胞介素(IL)-1、IL-2和IL-6的表达水平,用酶免疫分析法测定组胺的水平。结果治疗2周和4周后,西替利嗪组和协定方组有效率比较无统计学意义。治疗2周和4周后,联合用药组的有效率高于西替利嗪组和协定方组。与对照组比较,患者组的细胞因子IL-2和IL-6水平均明显增高。与患者组比较,治疗后西替利嗪组、协定方组、联合用药组的细胞因子IL-2和IL-6均明显降低。与西替利嗪组和协定方组比较,联合用药组的细胞因子IL-2和IL-6水平降低。与对照组比较,患者组的血清组胺水平明显增高。与患者组比较,治疗后西替利嗪组、协定方组、联合用药组的血清组胺水平均明显降低。与西替利嗪组和协定方组比较,联合用药组的血清组胺水平降低。结论本院自制口服中药协定方能有效治疗慢性荨麻疹。联合使用本院自制中药协定方加西替利嗪比单用疗效更好。其机制可能与降低慢性荨麻疹患者组胺、IL-2和IL-6的表达水平有关。Objective In order to investigate the efficacy of our hospital-made prescription of traditional Chinese medicine （TCM） in the treatment of patients with chronic urticaria,and to investigate the possible involved mechanism. Methods The patients were randomly divided into cetirizine group,prescription of TCM group and combined treatment group,the healthy people were taken as control group. The treatment efficacy was estimated by measuring the total scores after two weeks and four weeks treatment. Enzyme linked immunosorbent assay （ELISA） was used to detect the expression levels of cytokine IL-2 and IL-6. Enzyme immunoassay （EIA） was used to detect the expression levels of histamine. Results After treatment for two and four weeks,the total effective rate between cetirizine group and prescription of TCM group had no significant difference,while compared with that of cetirizine group or prescription of TCM group,the total effective rate of combined treatment group significantly were increased. Compared with those of control group,the levels of IL-2 and IL-6 of the patient were significantly increased. Compared with before treatment,the levels of IL-2 and IL-6 of the patients were significantly decreased after treatment. Compared with those of cetirizine group or prescription of TCM group,the levels of IL-2 and IL-6 of combined treatment group were significantly decreased. Compared with that of control group,the levels of histamine of the patient were significantly increased. Compared with before treatment,the levels of histamine of the patients were significantly decreased after treatment. Compared with that of cetirizine group or prescription of TCM group,the levels of histamine of combined treatment group were significantly decreased. Conclusion Hospital-made prescription of TCM can significantly improve clinical symptoms of the patients with chronic urticaria.
