摘 要:目的:探讨高流量鼻导管通气(HFNC)在急性呼吸衰竭患者经口气管插管中的应用价值。方法:将62例急性呼吸衰竭经口气管插管患者分两组,对照组采用常规方法插管,HFNC组在常规经口气管插管过程中加入HFNC(气流速度60 L/min、吸氧浓度100%)。比较两组插管过程中最低血氧饱和度及严重并发症(严重低氧血症、休克、心搏骤停、心率<60 min-1、室性心律失常、死亡)发生情况。结果:HFNC组患者插管过程中低氧血症程度低于对照组(P<0.05),2组严重并发症发生情况差异无统计学意义(P>0.05)。结论:经口气管插管过程中应用HFNC可显著改善急性呼吸衰竭患者的低氧血症。Aim:To explore the effect of high-flow nasal cannula oxygen( HFNC) therapy during endotracheal intubation in patients with acute respiratory failure. Methods:Sixty-two patients with acute respiratory failure were allocated into two groups. The control group was intubated by conventional method,and HFNC group were performed HFNC( airflow velocity of 60 L/min,oxygen concentration of 100%) during routine intubation. Minimum oxygen saturation and severe complications( severe hypoxemia,shock,cardiac arrest,heart rate 60 min-1,ventricular arrhythmia,death) were compared between the two groups. Results:The degree of hypoxemia in the HFNC group was lower than that in the control group(P〈0. 05),and there was no significant difference in the occurrence of serious complications between the two groups(P〈0. 05). Conclusion:The application of HFNC during endotracheal intubation could significantly improve the hypoxemia in patients with acute respiratory failure.
