摘  要:目的 探讨超高分辨率CT用于肺部小结节临床诊断中的应用价值.方法 选择2014年1月至2016年12月大同市第五人民医院收治且经病理确诊的肺部小结节(最大径≤2 cm)患者120例为研究对象,分别进行高分辨率CT扫描(矩阵为512×512)、超高分辨率CT扫描(矩阵为1024×1024),对两种分辨率CT扫描图像进行观察和分析,比较高分辨率CT扫描、超高分辨率CT扫描对肺部小结节的检出情况、CT征象检出情况、定性诊断结果、图像质量.结果 超高分辨率CT扫描对最大径为0.5~2 cm肺部小结节的检出个数与高分辨率CT扫描相比,差异无统计学意义(χ^2=0.695,P〉0.05),但超高分辨率CT扫描对最大径≤0.5 cm肺部小结节的检出个数多于高分辨率CT扫描(χ^2=6.021,P〈0.05).超高分辨率CT扫描对分叶征、毛刺征的检出率均高于高分辨率CT(χ^2=6.049、5.000,均P〈0.05),而在血管集束征、支气管征、胸膜凹陷征等CT征象的检出率,两种分辨率CT扫描差异均无统计学意义(χ^2=0.000、0.000、1.035,均P〉0.05).超高分辨率CT扫描对肺部恶性小结节、良性小结节的诊断符合率分别为98.08%、97.06%,均高于高分辨率CT扫描的86.54%、85.29%(χ^2=4.875、5.850,均P〈0.05).超高分辨率CT扫描的图像质量优良率为95.00%,高于高分辨率CT扫描的89.17%,但差异无统计学意义(χ^2=0.094,P〉0.05).结论 相比于常规高分辨率CT扫描,超高分辨率CT扫描对肺部小结节的图像质量更好,可对肺部小结节进行准确定性诊断,具有较高的诊断准确性,尤其是对最大径≤0.5cm的肺部小结节.Objective To explore the clinical value of ultra high resolution CT in the diagnosis of pulmonary nodules .Methods From January 2014 to December 2016 ,120 cases with small pulmonary nodules and confirmed by pathology(maximum diameter less than or equal to 2cm) in the Fifth People's Hospital of Datong were included in the study.The 120 patients received high resolution CT scan (512 ×512 matrix),ultra high resolution CT scan (1024 × 1024 matrix) .The images of two kinds of resolution CT were observed and analyzed ,compared with high-resolution CT scan,ultra high resolution CT scanning for detection of small pulmonary nodules ,CT signs detection situation, qualitative diagnosis,image quality.Results Compared with the high resolution CT scan ,ultra high resolution CT scan had no statistically significant differences in the detective rate of maximum diameter of 0.5 ~2cm small pulmonary nodules (χ^2 =0.695,P〉0.05),but the maximum diameter of the ultra high resolution CT scan less than or equal to 0.5cm small lung nodules detected more than that of the high resolution CT scan (χ^2 =6.021,P〈0.05). The detection rates of lobulation and spiculation of the ultra high resolution CT scanning were higher than those of high resolution CT (χ^2 =6.049,5.000,all P〈0.05),while the detective rates of the vascularconvergence sign ,bronchus sign,pleural indentation sign of the two kinds of resolution CT scan had no statistically significant differences (χ^2 =0.000,0.000,1.035,all P〉0.05).The diagnostic accordance rates of the ultra high resolution CT scan for pulmona -ry malignant nodules and benign nodules were 98.08%,97.06%,respectively,which were higher than those of the high resolution CT scan(86.54%,85.29%,χ^2 =4.875,5.850,all P〈0.05).The image quality rate of the ultra high resolution CT scanning was 95.00%,which was higher than 89.17% of the high resolution CT scan,but the difference was not statistically significant (χ^2 =0.094,P〉0.05).Conclusion Compared with the con
