不同规格可吸收缝线对自然分娩会阴切口缝合的美学效果及预后影响对比研究

摘　　要：目的:观察不同规格可吸收缝线用于自然分娩会阴切口缝合的美学效果及预后效果,为会阴切口缝合方案选择提供参考。方法:选择2015年2月-2016年1月在笔者医院自然分娩产妇208例作为研究对象,所有产妇均于局麻下行会阴切开生产,按照分娩顺序编号,将其随机分为对照组和研究组,每组104例。对照组采用3-0#可吸收缝线缝合,研究组采用5-0#可吸收免打结双向锯齿缝线(Quill缝线)缝合。两组产妇均采用会阴切口皮下美容缝合法缝合。两组产妇术后均由同一组护理人员实施护理。比较两组产妇缝合术后6h、24h、48h、72h疼痛评分,术后3d、7d、14d的线结反应,术后切口愈合等级、愈合时间、切口愈合相关并发症(裂开、硬结、切口针眼痕迹、外观瘢痕)等,并比较两组产妇对会阴切口缝合美学满意度调查。结果:会阴侧切口缝合术后研究组产妇6h、24h、48h、72h疼痛评分均明显低于对照组,差异有统计学意义(P<0.05)。术后3d、7d、14d研究组产妇线结反应0度率均显著高于对照组,线结反应Ⅲ度率明显低于对照组,差异有统计学意义(P<0.05)。研究组术后切口愈合时间、并发症发生率低于对照组,切口缝合外观美学满意率明显高于对照组,差异有统计学意义(P<0.05)。结论:采用相同美容缝合方式时,选择5-0#可吸收免打结双向锯齿缝线(Quill缝线)缝合会阴切口可获得更好的美学效果及切口愈合效果,降低切口愈合相关并发症率,提升产妇对缝合美学满意率。Objective To observe the aesthetic effect and prognosis effect of absorbable sutures with different sizes for natural delivery of perineal incision suture, so as to provide references for selection of suture schemes for perineal incision. Methods 208 cases of natural childbirth from February 2015 to January 2016 in our hospital were selected as the research objects. All the pregnant women were performed the vaginal incision under local anesthesia. According to the order of delivery, they were randomly divided into the control group and the observation group, 104 cases in each group. The pregnant women in the control group were sutured with 3-0# absorbable suture, and the patients in the observation group were sutured with 5-0# absorbable non knot double sawtooth suture （Quill suture）. The two groups of patients were sutured by perineal incision subcutaneous suture. The two groups of patients were treated by the same group of nursing staff. The two groups were compared after suturing of the 6h, 24h, 48h, 72h pain score, postoperative reaction 3d, 7d, 14d line, grade and healing complications incision healing time, postoperative wound healing （wound dehiscence, induration, needle marks, scars, and appearance） of two groups were compared on perineal incision suture aesthetic satisfaction survey. Results The pain scores of 6h, 24h, 48h and 72h in the observation group after perineal incision suture were signifcantly lower than those in the control group, and the difference were statistically signifcant（P〈0.05）. At 3, 7, and 14 days after operation, the rate of 0 degree of knot reaction in the observation group was signifcantly higher than that in the control group, and the rate of 3 degree of knot reaction in the observation group was signifcantly lower than that in the control group（P〈0.05）.
