补肾活血方联合针灸调经法治疗肾虚血瘀型排卵障碍性不孕症

摘　　要：目的:探讨补肾活血方联合针灸调经法治疗肾虚血瘀型排卵障碍性不孕症临床疗效及对患者卵泡发育、子宫内膜的影响。方法:选取符合纳入标准的126例肾虚血瘀型排卵障碍性不孕患者为研究对象,按照随机数表法分为治疗组与对照组,各63例,空白对照组选取门诊男方因素所导致的不孕患者63例。对照组患者给予补肾活血方,治疗组患者在此基础上加用针灸调经法,两组均进行3个疗程的治疗,空白对照组不接受任何治疗,结束后对患者临床疗效、卵泡发育与内膜生长、内分泌功能及子宫内膜容受性进行比较。结果:治疗组患者总有效率为90.48%(57/63),对照组为79.36%(50/63),差异有统计学意义(P<0.05);治疗后,两组患者均较治疗前成熟卵泡直径明显增大,子宫内膜厚度与雌二醇(Estradiol,E2)水平明显升高,子宫内膜螺旋动脉搏动指数(pulsatility index,PI)、阻力指数(resistance index,RI)、促黄体生成激素(luteotropic hormone,LH)与卵泡刺激素(follicle-stimulating hormone,FSH)水平明显降低,且治疗组显著优于对照组(P<0.05)。结论:补肾活血方联合针灸调经法可以明显促进肾虚血瘀型排卵障碍性不孕患者卵泡发育与内膜生长,改善子宫内膜容受性,调节患者生殖内分泌功能。Objective： To explore the clinical efficacy of Bushen Huoxue Formula combined with regulating menstruation acupuncture treating anovulatory infertility of Kidney-dificiency and blood-stasis and its effect on follicular development and endometrium. Methods： 126 cases of anovulatory infertility of Kidney-dificiency and blood-stasis were randomly divided into the control group（ n =63） and the treatment group（ n =63）. Another 63 cases of infertility due to partner problem were chosen as the blank control. The control group was treated with Bushen Huoxue Formula; on which basis, the treatment group was also treated with regulating menstruation acupuncture. The blank control group did not receive any treatment. The treatment effect, follicular development, endometrial growth, endocrine function, and endometrial receptivity were compared after three courses of the treatment in the three groups. Results： The total effective rate was 90.48%（57/63） in the treatment group, which was significantly higher than 79.36%（50/63） in the control group（ P 〈0.05）. After the treatment, the diameter of mature follicles were obviously increased, the endometrium was significantly thickened, the level of E 2 was greatly heightened, PI（pulsatility index） and RI（resistance index） of uterine helicine arteries were decreased, as well as the levels of LH and FSH were reduced in the treatment group and in the control group; of which the treatment group was superior to the control group（ P 〈0.05）. Conclusion： Bushen Huoxue Formula combined with regulating menstruation acupuncture can significantly promote follicular development and endometrial growth, and it can improve endocrine function and endometrial receptivity. It has better effect on anovulatory infertility.
