摘　　要：目的对白血病患儿的多症状进行特征分类,为针对性护理干预提供参考。方法采用一般资料调查表和中文版儿童自我报告结局测量信息系统中的儿童简表对218例白血病患儿进行横断面调查,采用潜类别模型根据患儿多症状特征及群体异质性进行特征分类。结果分为多症状轻组(42例)、多症状临界组(110例)和多症状困扰组(66例)3个亚组,其中多症状困扰组身体功能障碍程度显著高于另两组(均P<0.05),多症状轻组同伴关系显著优于多症状困扰组(P<0.05)。结论白血病患儿的多症状存在群体异质性,对个体造成不同程度的困扰;多症状3个亚组的划分可作为临床护理的参照。Objective To identify typologies（or clustering）of symptoms in children with leukemia and to provide references for targeted intervention.Methods A total of 218 children with leukemia were surveyed using a self-designed demographic questionnaire and the Pediatric Patient Reported Outcomes Measurement Information System（PROMIS）-Child Version.Latent class analysis（LCA）was performed to identify distinct groups based on symptom characteristics and population heterogeneity.Results The LCA identified 3 symptom profiles：mild symptom（n=42）,intermediate symptom（n=110）and severe symptom（n=66）.Children in ＂severe symptom class＂ had the worst physical functioning than those in the other two classes（P〈0.05 for both）,whereas children in ＂mild symptom class＂ had better peer relationship than those in ＂severe symptom class＂（P〈0.05）.Conclusion Symptom presentations in children with leukemia are heterogeneous and lead to different degrees of depressed feelings in children.Identifying latent subgroups may provide an empirical basis for practitioners to design and implement intervention efforts that are tailored to specific symptom profiles.
