摘 要:目的分析应用丹参注射液联合腺苷蛋氨酸对病毒性肝炎伴有黄疸患者肝功能的影响和安全性。方法随机选取自2013年2月—2018年2月接受治疗的重度病毒性肝炎患者共68例,按照随机列表法分为对照组(34例)与联合组(34例),对照组患者采用常规保肝治疗,而联合组患者采用丹参注射液和腺苷蛋氨酸联合疗法。观察并比较两组患者的治疗效果、肝功能指标(ALT、ALB、ALP、AST、DBIL)与不良反应率。结果联合组患者的治疗总有效率(97.06%)比对照组的(76.47%)明显增加,组间差异有统计学意义(χ~2=6.275,P=0.012);治疗后,联合组患者的血清白蛋白(albumin,ALB)[(50.13±8.04)g/L]都比对照组的[(37.10±7.86)g/L]明显增加,组间差异有统计学意义(t=6.757,P=0.009);联合组患者的碱性磷酸酶(Alkaline phosphatase,ALP)、谷草转氨酶(glutamic oxaloacetic transaminase,AST)和直接胆红素(Direct bilirubin,DBIL)指标都比对照组的明显降低,组间差异有统计学意义(t=13.514,6.973,6.189,P=0.000,0.005,0.013);两组患者的不良反应率(8.82%vs11.76%)差异无统计学意义(χ~2=0.159,P=0.690)。结论应用丹参注射液联合腺苷蛋氨酸能有效地提高病毒性肝炎患者的疗效,改善患者的肝功能,且不会增加患者不良反应率,安全性高。Objective To analyze the effect and safety of Danshen injection combined with adenosylmethionine on liver function in patients with viral hepatitis and jaundice. Methods A total of 68 patients with severe viral hepatitis who were treated from February 2013 to February 2018 were randomly selected. The patients were divided into the control group (34 cases) and the combined group (34 cases) according to the randomized list. Conventional liver protection treatment, and the combined group of patients with Danshen injection and adenosyl methionine combination therapy. The therapeutic effects, liver function indexes (ALT, ALB, ALP, AST, DBIL) and adverse reaction rates were observed and compared between the two groups. Results The total effective rate of treatment in the combined group (97.06%) was significantly higher than that in the control group (76.47%). The difference between the two groups was statistically significant (χ^2=6.275, P=0.012). After treatment, the serum of the combined group was white. The protein (albumin, ALB) [(50.13±8.04)g/L] was significantly in- creased compared with the control group[(37.10±7.86)g/L], and the difference between the groups was statistically significant (t=6.757, P=0.009); Alkaline phosphatase (ALP), glutamic oxaloacetic transaminase (AST) and direct bilirubin (DBIL) were significantly lower in the combination group than in the control group. The difference was statistically significant (t=13.514, 6.973, 6.189, P=0.000, 0.005, 0.013),there was no statistically significant difference in the adverse reaction rate (8.82% vs 11.76%) between the two groups (χ^2=0.159, P=0.690). Conclusion The application of salvia mihiorrhiza injection combined with adenosylmethionine can effectively improve the efficacy of patients with viral hepatitis and improve the liver function of patients without increasing the adverse reaction rate and safety.
