腹腔镜疝修补术与开放式无张力疝修补术治疗成人腹股沟疝的临床比较

摘　　要：目的探析腹腔镜疝修补术与开放式无张力疝修补术分别用于成人腹股沟疝的临床治疗效果。方法方便选取该院2016年1月—2018年4月收治的112例腹股沟疝的成人患者,随机分为对照组和研究组,每组各56例,其中对照组采用开放式无张力疝修补术进行治疗,研究组采用腹腔镜疝修补术进行治疗,比较两组患者的临床治疗效果、手术相关情况以及并发症情况。结果研究组总有效率为91.07%,明显高于对照组(χ~2=7.473,P<0.05),研究组手术时间(45.2±10.5)min与对照组比较差异无统计学意义(t=1.242,P>0.05);研究组术中出血量为(25.7±3.6)m L、术后疼痛评分(2.1±0.2)分、恢复活动时间(157.28±26.37)h、住院时间为(3.7±1.6)d,明显优于对照组(t=4.29,4.67,7.63,4.33,P<0.05)。研究组的并发症发生率(8.33%)低于对照组(χ~2=6.892,P<0.05)。结论腹股沟疝的成人患者采用腹腔镜疝修补术治疗的综合效果较好,临床有效率高、术后各项指标较好,且并发症发生率低,在临床治疗中具有重要的借鉴价值。Objective To investigate the clinical effect of laparoscopic hernia repair and open tension-free hernia repair for adult inguinal hernia. Methods 112 adult patients with inguinal hernia admitted from January 2016 to April 2018 were convenient selected and divided into control group and study group, 56 cases in each group. The control group was treated with open tension-free hernia repair. The study group was treated with laparoscopic hernia repair, and the clinical treatment effect, surgical related conditions and complications were compared between the two groups. Results The total effective rate of the study group was 91.07%, which was significantly higher than that of the control group （X^2=7.473, P〈0.05）. The operation time of the study group （45.2±10.5）min was not significantly different from the control group （t=1.242, P〉0.05）. The intraoperative blood loss was （25.7±3.6）mL, postoperative pain score （2.1±0.2）points, recovery time （157.28±26.37）h, and hospitalization time （3.7±1.6） d, which was significantly better than the control group points（t=4.29, 4.67, 7.63, 4.33, P〈0.05）. The complication rate of the study group （8.33%） was lower than that of the control group （X^2=6.892, P〈0.05）. Conclusion Adult patients with inguinal hernia have better comprehensive effect with laparoscopic hernia repair, high clinical efficiency, good postoperative indexes, and low complication rate. It has important reference value in clinical treatment.
