肱骨近端复杂性骨折应用半肩关节置换治疗的价值分析

摘　　要：目的对肱骨近端复杂性骨折采取人工半肩关节置换术治疗的临床效果进行探析。方法方便选取2015年10月—2017年10月该院骨外科收治的97例肱骨近端复杂性骨折患者的临床资料进行回顾性分析,根据治疗方法不同分成研究组和对照组,分别为55例和42例;对照组采取常规锁定钢板内固定术治疗,研究组采取人工半肩关节置换术治疗,对比分析两组疗效。结果研究组手术时间、术中出血量低于对照组(t=2.019、1.003,P<0.05);术后随访6个月,研究组患肢功能恢复优良率为96.36%,对照组为85.72%,差异有统计意义(χ~2=10.287,P<0.05);研究组术后并发症发生率为2.38%,低于对照组的14.29%(χ~2=2.087,P<0.05)。结论采用半肩关节置换术治疗肱骨近端复杂性骨折有着理想效果,手术时间短,术中出血少,术后并发症少,预后良好,有着重要临床价值。Objective To analyze the clinical effect of artificial semi-shoulder arthroplasty for the treatment of complex proximal humerus fractures. Methods The clinical data of 97 patients with proximal humeral complex fractures admitted to the department from October 2015 to October 2017 were convenient selected and retrospectively analyzed. According to different treatment methods, they were divided into study group and control group, 55 cases and 42 cases; the control group was treated with conventional locking plate internal fixation, the study group was treated with artificial half-shoulder joint replacement, and the two groups were compared and analyzed. Results The operation time and intraoperative blood loss in the study group were lower than those in the control group （t=2.019, 1.003 ,P〈0.05）. After 6 months of follow-up, the excellent and good rate of limb function recovery in the study group was 96.36%, and the control group was 85.72%, the difference was statistically significant （X^2=10.287,P〈0.05）; the postoperative complication rate was 2.38% in the study group, which was lower than 14.29% in the control group （X^2=2.087,P〈0.05）. Conclusion The use of semi-shoulder arthroplasty for the treatment of complex proximal humerus fractures has a good effect, with short operation time, less intraoperative blood loss, less postoperative complications and good prognosis, which has important clinical value.
