基于提高ICU患者非计划拔管预防措施落实率的品管圈实践

摘　　要：目的探讨"生命延续圈"品管圈活动在提高ICU患者非计划拔管预防措施落实率中的应用效果。方法 2017年9月—2018年4月,在北大医疗鲁中医院ICU组建"生命延续圈",开展"以提高患者非计划拔管预防措施落实率"为主题的品管圈活动,比较活动前后有形成果、无形成果。结果实施"生命延续圈"活动后,非计划拔管措施落实率为85.73%,显著高于实施前69.47%,差异有统计学意义(P<0.01);目标达标率为110.40%,进步率为24.20%。团队精神、沟通能力、积极性、自信心、创新能力、品管手法、责任感评分均为正向成长。结论 "生命延续圈"有助于提高ICU患者非计划拔管预防措施落实率,增强护理人员团队精神及责任感,对提升护理质量具有重要意义。Objective To study the application effect of ＂life continuum circle＂ quality control circle in improving the preventive measures for unplanned extubation of ICU patients. Methods The life continuum circle in ICU in our hospital from September 2017 to April 2018 was composed, and the quality control circle taking the ＂improving the preventive measures for unplanned extubation of ICU patients＂ as the theme, and the tangible results and intangible results were compared before and after the quality control circle. Results After the implementation, the implementation rate of unplanned extubation measures was obviously higher than that before the implementation（85.73% vs 69.47%）,the different was statistically significant（P0.01）, and the target standards-reaching rate and progress rate were respectively 110.40% and 24.20%, and the team spirit, communication ability, enthusiasm, confidence, innovation ability, quality control technology, and score of responsibility sense showed a positive growth. Conclusion The ＂life continuum circle＂ contributes to improving the implementation rate of preventive measures for unplanned extubation of ICU patients, and enhancing the team spirit and sense of responsibility of nursing team, which is of important significance to improving the nursing quality.
