慢性肺源性心脏病合并多脏器功能衰竭50例临床分析

摘　　要：目的探讨慢性肺源性心脏病合并多脏器功能衰竭临床诊治体会。方法回顾性分析2015年1月—2017年12月该院诊治的50例慢性肺源性心脏病合并多脏器功能衰竭患者临床资料,观察多脏器功能损害情况,治疗及结果。结果 50例患者共发生不同脏器功能衰竭134例次,经过综合治疗,针对各脏器衰竭处理,34例监测指标逐渐恢复正常,好转出院,住院时间(23.68±5.08)d,16例经抢救无救死亡。其中呼吸衰竭受累率49例(98.00%),心衰竭31例(62.00%);脑功能受损22例(44.00%);肝肾功能受损20例,消化道出血11例(22.00%),血液系统受损1例(2.00%)。2个脏器衰竭28例(56.00%),死亡率14.29%;3个脏器衰竭12例(24.00%),死亡率33.33%;4个脏器衰竭8例(死亡率75.00%);5个脏器衰竭2例(死亡率100.00%)。结论慢性肺源性心脏病并发多脏器功能衰竭的发生率高,对预后有明显不良影响,应早期诊断积极,同时实施针对性措施预防多脏器功能衰竭的发生。Objective To study the diagnosis and treatment experience of chronic pulmonary heat disease combined with muhiple organ failure. Methods 50 cases of patients with chronic pulmonary heat disease combined with muhiple organ failure admitted and treated in our hospital from January 2015 to December 2017 were selected, and the impair ment, treatment and results of muhiple organ failure were observed. Results Of 50 cases, there were 134 with different organ failure, after the comprehensive treatment, the monitoring indexes of 34 cases returned to normal and discharged after the improvement, and the length of stay was （23.68＋5.08）d, and 16 cases died after the invalid rescue, including 49 cases involved with respiration failure （98.00%）, 31 cases with heart failure （62.00%）, 22 cases with cerebral func tion injury （44.00%）, 20 cases with injmT of liver and renal functions, 11 cases with digestive tract hemorrhage （22.00%）, 1 case with blood system injury（2.00%）, 28 cases with two organs injury（56.00%）, the morbidity was 14.29%; 12 cases with three organs failure （24.00%）, the morbidity was 33.33%, 8 cases with four organs failure, and the mor bidity was 75.00%, 2 cases with five organs failure, and the morbidity was 100.00%. Conclusion The incidence rate of chronic pulmonary heat disease combined with muhiple organ failure is high, which has an obvious adverse effect on the prognosis, and we should actively diagnose them and implement the targeted measures to prevent the occurrence of muhiple organ failure.
