铁皮石斛对慢性肾炎患者外周血Th1/Th2及CD28^+细胞水平的影响

摘　　要：为探讨铁皮石斛对慢性肾炎患者外周血Th1/Th2及CD28^+细胞水平的影响,本实验选取慢性肾炎患者60例,随机分为观察组和对照组各30例,并选择20例健康体检者作为健康组。在常规治疗基础上,对照组给予替米沙坦治疗,观察组在对照组基础上加服铁皮石斛,两组患者连续治疗3个月。比较两组患者治疗前后的外周血IFN-γ、IL-4、Th1/Th2、CD28^+、CD4^+CD28^+、CD8^+CD28^+细胞、血清胱抑素(cystatin C, CysC)、β_2微球蛋白及24h尿蛋白定量(24hurinary protein, Upr)水平,同时与健康组作比较。研究结果显示治疗前两组患者外周血IFN-γ、IL-4、Th1/Th2、CD28^+、CD4^+CD28^+、CD8^+CD28^+细胞、CysC、β_2微球蛋白及Upr水平比较,差异无统计学意义(P> 0.05),与健康组比较,差异有统计学意义(P<0.05);治疗后两组患者IFN-γ、Th1/Th2、CD8^+CD28^+细胞、CysC、β_2微球蛋白及Upr水平降低(P<0.05),IL-4、CD28^+、CD4^+CD28^+细胞水平升高(P<0.05),且观察组优于对照组,差异有统计学意义(P<0.05)。综上,本研究发现铁皮石斛能够调节患者的免疫功能,改善慢性肾炎患者肾功能指标。To study the effects of Dendrobium officinale on the serum levels of Th1/Th2 and CD28^＋ cells in patients with chronic nephritis,60 patients with chronic nephritis were randomly divided into the observation group（n=30）and the control group（n=30）,and 20 healthy persons were selected as the healthy group.In addition to the conventional therapy for the observation group and the control group,Telmisartan was given to patients of the control group,and Dendrobium officinale in addition to Telmisartan was given to the patients of observation group.Both groups were treated for 3 months.The levels of serum IFN-γ,IL-4,Th1/Th2,CD28^＋,CD4^＋CD28^＋,CD8^＋CD28^＋ cells,CysC,β_2 microglobulin and Upr were measured before and after treatment and were compared between the two groups,then compared with the healthy group.The results showed that there was no statistically significant differences in serum IFN-γ,IL-4,Th1/Th2,CD28^＋,CD4^＋CD28^＋,CD8^＋CD28^＋ cells,CysC,β2 microglobulin and Upr levels between the observation group and the control group before treatment（P〉0.05）,while compared with the healthy group,the difference was statistically significant（P〈0.05）.After treatment,the levels of IFN-γ,Th1/Th2,CD8^＋CD28^＋ cells,CysC,β2 microglobulin and Upr were significantly decreased（P〈0.05）,the levels of IL-4,CD28^＋,and CD4^＋CD28^＋ cells were significantly increased（P〈0.05）between the observation group and the control group,the effective rate in the observation group was higher than that of the control group,the difference had statistical significance（P〈0.05）.This study found that Dendrobium officinale can improve the renal function of patients with chronic nephritis and regulate the immune function of patients,suggesting that it is worthy of clinical application.
