摘 要:目的:研究并探讨应用注射用白眉蛇毒血凝酶对颅脑手术中头皮切口的止血作用、对凝血功能的影响及药物安全性。方法:60例神经外科需行开颅手术的患者随机分为治疗组和对照组。治疗组给予白眉蛇毒血凝酶,术前1晚肌注1 u,术前1 h肌注1 u,术前15 min静注1 u。对照组给予等体积0. 9%氯化钠注射液,给药时间及方式均同治疗组。比较两组患者头皮手术切口的出血量和单位面积出血量、止血时间,观察两组患者血小板计数和凝血功能变化,及用药安全性。结果:治疗组切口平均出血量、切口平均单位面积出血量、平均止血时间均明显少于对照组(P <0. 05)。术前及术后各时间点,两组患者血小板计数及凝血酶原时间(、凝血酶时间、活化部分凝血活酶时间、纤维蛋白原含量等各项指标无明显变化,组内、组间比较差异无统计学意义(P>0. 05)。患者术前术后安全性指标均无明显异常改变,未出现任何不良事件。结论:注射用白眉蛇毒血凝酶对头皮手术切口毛细血管出血有较好的止血作用,具有较高的安全性,有助于颅脑手术顺利进行并缩短手术时间。Objective: To investigate the hemostasis effect of hemocoagulase for injection in scalp incision in craniocerebral surgery,and evaluate its effect on coagulation function and drug safety. Methods: Before undergoing craniotomy,60 patients were randomly divided into the study group and the control group. The study group was injected with hemocoagulase for injection at 1 u im at the night before surgery,1 u im 1 h before incision and 1 u im 15 min before incision. The control group was injected with 0. 9% saline at the same time with the same volume. The hemorrhagic volume,hemorrhagic volume per square decimeter,hemostatic time,blood coagulation and adverse events were tested and compared between the groups. Results: In the study group,the mean hemorrhagic volume was( 37. 18 ±2. 96) g,the mean hemorrhagic volume per square decimeter was( 0. 23 ± 0. 16) g·cm-(-2) and the mean hemostatic time was( 125. 53 ±36. 42) s. In the control group,the corresponding value was( 60. 69 ± 2. 30) g,( 0. 42 ± 0. 25) g·cm-(-2) and( 182. 72 ± 52. 29) s,respectively. The hemorrhagic volume,hemorrhagic volume per square decimeter and hemostatic time significantly decreased in the study group when compared with those in the control group( P〈0. 05). No significant difference in blood coagulation and safety( P〉0. 05).No adverse event was reported. Conclusion: Hemocoagulase for injection shows promising hemostasis effect with high safety,which can ensure craniocerebral surgery going well with shortened operation time.
