摘  要:目的探讨针刺治疗青年原发性多汗症的临床疗效及皮肤交感反应(SSR)检测的变化,阐明青年原发性多汗症患者的交感神经纤维损害特征及针刺的治疗作用。方法选用27例原发性多汗症患者和30例健康对照者作为研究对象,选合谷、劳宫、曲泽、三阴交、复溜、大杼、风门、肺俞、厥阴俞等腧穴对多汗症组进行针刺治疗。10 d为1个疗程,每2个疗程隔2 d,治疗8个疗程。治疗前后均进行SSR检测,记录四肢SSR的起始潜伏期(Lat)和峰-峰波幅(Amp)。结果 1)临床疗效观察结果:研究组明显改善为10例(10/27,37.0%),轻度改善14例(14/27,51.9%),无明显改善3例(3/27,11.1%),总有效24例(24/27,88.9%)。2)SSR检测:健康对照组SSR,上肢Lat(1.49±0.27),下肢Lat(2.08±0.35);多汗症组SSR,上肢Lat(1.62±0.14),下肢Lat (2.35±0.10).多汗症组四肢Lat延长,异常率为8例(29.6%)。健康对照组SSR,上肢Amp (9.13±3.16),下肢Amp(3.20±1.37);多汗症组SSR,上肢Amp(5.33±2.55),下肢Amp (1.16±0.46);多汗症组四肢Amp降低,异常率为8例(29.6%)。经过8个疗程的治疗后,与治疗前比较,四肢Lat缩短,四肢Amp升高,研究组SSR均恢复正常。结论针刺上述腧穴对青年原发性多汗症有较好的治疗作用,其作用机制可能与改善交感神经纤维功能有关。SSR检测有助于发现青年原发性多汗症患者交感神经纤维亚临床损害及疗效评价。Objective To study the effect of acupuncture treatment and the using of Sympathetic skin response(SSR) in young patients with Primary hyperhidrosis(PH). Methods 27 young patients with PH and 30 health controls were retrospectively reviewed. Hegu(LI 4), Laogong(PC 8), Quze(PC 3), Sauyinjiao(SP 6), Fuliu(KI 7), Dazhu(BL11), Fengmen(BL 12), Feishu(BL 13), Jueyinshu(BL 14) were selected as the acupucture sites. Every patients were treated 8 courses(10 days as a course, and an interval of 2 days between courses). Latency and amplitude of SSR were studied before and after the treatments. Results(1) the effects of acupuncture: 10(10/27, 37.0%) had aignifi cant improvements, 14(14/27, 51.9%) had mild improvements and 3(3/27, 11.1%)hadn't improvement.2) SSR: latency in upper limbs were(1.49±0.27) and(1.62±0.14), and latency in lower limbs were(2.08±0.35) and Lat(2.35±0.10)in controls and patients, respectively. Prolonged latency was seen in 8(29.6%) patients. amplitude in upper limbs were(9.13±3.16) and(5.33±2.55), and amplitude in lower limbs were(3.20±1.37) and Lat(1.16±0.46) in controls and patients, respectively. Lower amplitude was seen in 8(29.6%) patients. After the treatments, latency and amplitude were normal in all patients. Conclusion Acupuncture is helpful in young patients with PH, and it is related to the improvement of the sympathetic nerve. SSR is useful to detect the function of sympathetic nerve in patients with PH and can evaluate the effects of acupuncture.
