摘 要:目的探讨出血性中风患者瘀热证与炎症、凝血之间的相关性。方法选择出血性中风瘀热证与非瘀热证患者各9例分别组成瘀热组和非瘀热组。收集血液样本,提取淋巴细胞,应用高通量测序技术测定两组基因表达,分析两组差异表达的基因,对差异表达基因进行功能和通路分析。结果两组基因表达谱存在明显差异,瘀热组与非瘀热组相比较,表达上调≥2倍的基因2867条,表达下调≥2倍的基因有1877条;差异表达的基因涉及的通路有凝血相关通路、炎症免疫相关通路、血管内皮相关通路、细胞凋亡与细胞周期相关通路等;实时定量PCR验证瘀热组与非瘀热组肿瘤坏死因子受体超家族成员18(TNFRSF18)、基质金属蛋白酶家族23B(MMP23B)、DNAJ同源B亚家族成员14(DNAJB14)、白细胞介素2受体α亚基(IL2RA)、C1Q肿瘤坏死因子3(C1QTNF3)、人血小板生长因子(PDGFRB)、凝血因子Ⅷ(F8)和凝血因子Ⅱ(F2)基因,差异表达结果与基因芯片完全一致。结论出血性中风瘀热证的本质与凝血、炎症病理的相关,因血行凝滞不畅导致"瘀",机体炎性反应导致"热"可能是其本质。Objective To explore the correlation between stagnated heat syndrome and clotting,inflammation in hemorrhagic stroke patients. Methods Hemorrhagic stroke patients with stagnated heat syndrome and non-stagnated heat syndrome were selected and divided into stagnated heat group and non-stagnated heat group,with 9 patients in each group. The blood samples of both groups were collected to extract lymphocytes. High-throughput sequencing was applied to detect gene expressions. The functions and pathways of the differentially expressed genes were analyzed.Results There was significant difference in gene expression spectrum of both groups. Compared with the non-stasis heat group,there were 2867 genes whose expression was up-regulated ≥2 times,and 1877 genes whose expression was down-regulated ≥2 times. The pathways that related to differentially expressed genes were coagulation-related pathways,inflammatory immune-related pathways,vascular endothelial pathways,apoptosis,and cell cycle related pathways. Real-time PCR was used to verify the expression of tumor necrosis factor receptor superfamily 18( TNFRSF18),matrix metalloproteinase 23 B( MMP23 B),DNAJ homologous B subfamily 14( DNAJB14),interleukin 2 receptor α( IL2 RA),C1 Q tumor necrosis factor 3( C1 QTNF3),pleteletdericed growth factor( PDGFRB),blood coagulation factor Ⅷ( F8) and blood coagulation factor Ⅱ( F2),and the results were consistent with the gene chip. Conclusion The essential feature of hemorrhagic stroke of stagnated heat syndrome is related to clotting and inflammation. Stasis elicited by stagnation of blood and heat caused by inflammatory reaction might be the nature of hemorrhagic stroke with stagnated heat syndrome.
