摘　　要：目的研究右美托咪定用于子宫下段剖腹产椎管麻醉镇静的效果。方法选择2017年4月至2018年4月在我院接受子宫下段剖腹产的276例患者为观察对象,所有患者均接受椎管麻醉镇静,同时采用分组对比法,给予随机分组,包括对照组与观察组,各138例,对照组给予药物咪达唑仑,观察组给予药物为右美托咪定,对比分析不同麻醉药物在椎管麻醉过程中的实际效果,根据其麻醉质量进行应用价值及可行性分析。结果两组患者用药前Ramsay评分、HR、SBP、DBP值对比无明显差异,用药5min后其指标存在一定程度改善,用药15min及停药后其Ramsay评分、HR、SBP、DBP值对比存在明显差异,观察组各指标水平显著优于对照组P <0.05,差异有统计学意义。结论右美托咪定应用于子宫下段剖腹产椎管麻醉镇静的过程中效果显著,持续时间更长,安全性更高,具有十分积极的临床研究价值,值得推广。Objective To study the effect of dexmedetomidine on sedation of spinal anesthesia in the lower uterus. Methods From April 2017 to April 2018, 276 cases of lower uterine caesarean section in our hospital were selected as the observation subjects. All patients received spinal anesthesia and sedation. At the same time, the group comparison method was used to give the random group, including the control group and the observation group, 138 cases in each group. The control group was given midazolam, while the observation group was given dexmedetomidine. To compare and analyze the actual effects of different anesthetics in spinal canal anesthesia, and to analyze the application value and feasibility according to the anesthetic quality. Results There was no significant difference in Ramsay score, HR, SBP and DBP between the two groups. There was no significant difference in Ramsay score, HR, SBP and DBP between the two groups before treatment, but the indexes were improved to some extent after 5 minutes of treatment. There were significant differences in Ramsay score, HR, SBP and DBP between the two groups after 15 minutes of treatment and after withdrawal. The level of each indicator in the observation group was significantly better than that in the control group P〈0.05, and the difference was statistically significant. Conclusion Dexmedetomidine should be used in the process of sedation under caesarean section of lower uterine segment. It has a longer duration and higher safety. It has a very positive clinical research value and is worth promoting.
