逆行气管插管在困难气道紧急插管中的研究应用

摘　　要：目的探讨逆行气管插管在困难气道紧急插管中的应用效果。方法回顾性选择2014年8月至2018年6月入院治疗的困难气道紧急插管患者28例,所有患者均行常规方法治疗,包括:纤支镜,光索,喉罩等,但是未能取得理想的效果,患者最终采用逆行气管插管治疗,分析逆行气管插管在困难气道紧急插管患者中的临床效果。结果 28例困难气道紧急插管患者均顺利完成逆行气管插管治疗,27例取得治疗成功,成功率为96.00%,1例患者均由于导丝打折,吸痰引导管不能准确进入气管内,后来经过更换导丝,1例病人都完成了气管插管,无一例失败,也无严重并发症发生。结论将逆行气管插管用于困难气道紧急插管中效果理想,能保证患者有效通气,最大程度地减少困难气道紧急插管带来的风险,为手术创造有利条件,值得推广应用。Objective to explore the application effect of retrograde tracheal intubation in emergency intubation of difficult airway. Methods 28 patients with difficult airway emergency intubation who were admitted to the hospital from August 2014 to June 2018 were retrospectively selected. All patients were treated with conventional methods, including fibrobronchoscope, optical cable and laryngeal mask. However, the ideal effect was not achieved, and the patient was finally treated with retrograde endotracheal intubation to analyze the clinical effect of retrograde endotracheal intubation in patients with difficult airway emergency intubation. Results all the 28 patients with difficult airway emergency intubation successfully completed the retrograde endotracheal intubation, and 27 patients were successfully treated with 96.00% success rate. 1 patients could not enter the trachea accurately due to the discount of the guidewire, and after the replacement of the guidewire, the intubation was completed in 1 patient, without any failure or serious complications. Conclusion The retrograde tracheal intubation is ideal for difficult airway emergency intubation, which can ensure the effective ventilation of patients, minimize the risk caused by emergency intubation of difficult airway, and create favorable conditions for operation. It is worth popularizing and applying.
