左炔诺孕酮宫内缓释系统治疗无排卵性功能失调性子宫出血的疗效观察

摘　　要：目的:探讨左炔诺孕酮宫内缓释系统治疗无排卵性功能失调性子宫出血的疗效。方法:选取2015年11月至2017年11月江西省妇幼保健院收治的无排卵性功能失调性子宫出血患者82例,根据抛硬币法随机分组原则分为观察组和对照组,每组41例。对照组患者给予常规治疗,观察组患者给予左炔诺孕酮宫内缓释系统治疗。比较两组患者痛经评分、月经量评分、血脂水平[总胆固醇(TC)、三酰甘油(TG)、高密度脂蛋白胆固醇(HDL-C)及低密度脂蛋白胆固醇(LDL-C)]及生殖激素水平[卵泡刺激素(FSH)、黄体生成素(LH)、雌二醇(E_2)及孕酮(P)]的差异。结果:治疗后,观察组患者痛经评分、月经量评分均明显优于治疗前及对照组治疗后,TC、TG、HDL-C及LDL-C水平明显优于治疗前及对照组治疗后,E2、P水平明显高于对照组,上述差异均有统计学意义(P<0.05);而观察组患者FSH、LH水平与治疗前及对照组治疗后的差异均无统计学意义(P>0.05)。结论:左炔诺孕酮宫内缓释系统治疗无排卵性功能失调性子宫出血的疗效显著。OBJECTIVE： To probe into the efficacy of levonorgestrel intrauterine sustained release system in treatment of anovulation dysfunctional uterine bleeding. METHODS： 82 patients with anovulation dysfunctional uterine bleeding admitted into Jiangxi Maternal and Child Health Hospital from Nov. 2015 to Nov. 2017 were extracted to be divided into observation group and control group via the toss coining method,with 41 cases in each. The control group was treated with conventional treatment,while the observation group received levonorgestrel intrauterine sustained release system. Differences of dysmenorrhea score,menstrual blood score,blood lipid levels [total cholesterol（ TC）,triglyceride（ TG）,high-density lipoprotein cholesterol（ HDL-C） and low-density lipoprotein cholesterol（ LDL-C） ]and reproductive hormone levels [Follicle stimulating hormone（ FSH）,luteinizing hormone（ LH）,estradiol（ E2）and progesterone（ P） ]of two groups were compared. RESULTS： After treatment,the dysmenorrhea score,menstrual blood score of observation group were significantly better than before treatment and of control group after treatment,the TC,TG,HDL-C and LDL-C of observation group were significantly better than before treatment and of control group after treatment,the E2 and P level of observation group was significantly higher than that of control group,with statistically significant difference（ P〈0. 05）. There was no significant difference in FSH and LH levels between the observation group and before treatment,the control group after treatment（ P〈0. 05）. CONCLUSIONS： The efficacy of levonorgestrel intrauterine sustained release system in treatment of anovulation dysfunctional uterine bleeding is remarkable.
