摘　　要：目的:探讨临床药师对体位性低血压患者实施药学监护的方法。方法:临床药师深入临床,针对1例体位性低血压患者,根据治疗原则建立个体化的药学监护计划,建议医师将易导致体位性低血压的盐酸特拉唑嗪更换为盐酸坦索罗辛,同时根据患者血压逐渐减少抗高血压药用量,对患者进行个体化药学监护。结果:通过药学监护,患者立、卧位血压恢复正常,保障了用药安全。结论:体位性低血压患者属于心内科临床药师的重点监护对象,鉴别何种药物导致体位性低血压,并通过药学监护改善患者血压是心内科临床药师必不可少的能力之一,可提高患者用药安全性和依从性。OBJECTIVE： To probe into the method of pharmaceutical care for patients with postural hypotension conducted by clinical pharmacists. METHODS： Clinical pharmacist went deep into the clinic to formulate individualized pharmacological monitoring program for a patient with postural hypotension according to treatment principle,and suggested physicians to replace the terazosin hydrochloride, which was easy to cause postural hypotension, with tamsulosin hydrochloride. At the same time, clinical pharmacists performed individualized pharmaceutical supervision to gradually reduce the usage amount of antibypertensive drugs according to the patient＇s blood pressure. RESULTS： Through the pharmaceutical care,patient＇s standing and supine blood pressure had returned to normal,which ensured the safety of drug application. CONCLUSIONS： Patients with postural hypotension are the key monitoring subjects of clinical pharmacists in cardiology department. To identify the drugs cause of postural hypotension and improve patients＇ blood pressure through pharmaceutical care are one of the indispensable abilities for clinical pharmacists in cardiology department,which can improve patients＇ medication safety and compliance.
