小剂量多巴胺联合呋塞米静脉泵入治疗顽固性心力衰竭的疗效观察

摘　　要：目的:探讨小剂量多巴胺联合呋塞米静脉泵入治疗顽固性心力衰竭的临床疗效。方法:选取2016年1月至2018年2月天津医科大学总医院滨海医院收治的顽固性心力衰竭患者60例,以随机抽取形式分为观察组和对照组,每组30例。对照组患者给予呋塞米静脉泵入治疗,观察组患者在对照组的基础上加用小剂量多巴胺。比较两组患者的临床疗效、心功能指标的差异。结果:观察组患者的总有效率为90.00%(27/30),明显高于对照组的66.67%(20/30),差异有统计学意义(P<0.05);治疗后,观察组患者左心室射血分数、左心室舒张末期内径及血浆N末端B型脑钠肽原水平明显优于对照组,差异均有统计学意义(P<0.05)。结论:采用小剂量多巴胺联合呋塞米静脉泵入治疗顽固性心力衰竭,在有效提高疗效的同时有利于患者心功能的改善。OBJECTIVE： To probe into the clinical efficacy of low-dose dopamine combined with intravenous infusion of furacemi on refractory heart failure. METHODS： 60 patients with refractory heart failure admitted into Tianjin Medical University General Hospital Binhai Hospital from Jan. 2016 to Feb. 2018 were extracted to be randomly divided observation group and control group,with 30 cases in each. The control group was given intravenous infusion of furacemi,while the observation group additionally received low-dose dopamine based on the control group.Differences of clinical efficacy,cardiac parameters of two groups were compared. RESULTS： The total effective rate of observation group was 90. 00%（ 27/30）,significantly higher than that of control group（ 66. 67%,20/30）,with statistically significant difference（ P〈0. 05）. After treatment,the left ventricular ejection fraction,left ventricular end diastolic diameter and plasma N-terminal B-type pro-brain natriuretic peptide in observation group were significantly better than those in control group, and the differences were statistically significant（ P〈0. 05）.CONCLUSIONS： Low-dose dopamine combined with intravenous infusion of furacemi on refractory heart failure can not only improve the efficiency of treatment but also improve the cardiac function.
