摘  要:目的:观察自拟补肾活血汤对糖尿病周围血管神经病变患者糖脂代谢、神经传导速度及血清因子水平的影响。方法:将我院收治的糖尿病周围血管神经病变患者72例随机分为对照组和观察组患者各36例。对照组患者在常规降糖、降脂等基础上给予西洛他唑片口服;观察组患者则在对照组的基础上加用自拟补肾活血汤内服,两组均连续治疗8周。比较两组的治疗效果,观察患者治疗前后血糖、血脂水平,监测神经传导速度及血清同型半胱氨酸(Hyc)、转化生长因子β1 (TGF-β1)水平。结果:观察组有效率为91. 7%,明显高于对照组的有效率为75. 0%(P<0. 05);观察组的血糖FPG、2hBG、Hb A1c及血脂TC、TG、LDL-C等水平与对照组相比显著降低(P<0. 05);观察组患者经治疗后正中神经、腓总神经的神经传导速度SNCV、MNCV较对照组显著升高(P<0. 05);治疗后两组患者的Hcy、TGF-β1水平较治疗前显著降低,观察组指标降低尤其明显(P<0. 05)。结论:自拟补肾活血汤治疗糖尿病周围血管神经病患者疗效肯定,利于改善糖脂代谢,提高神经传导速度,其机制与调控血清中Hyc、TGF-β1水平以降低神经元损害有关,值得临床推广应用。Objective: To analyze the effect of treating diabetic peripheral vascular neuropathy patients with nourishing kidney and activating blood methods. Methods: 72 cases were randomly divided into a controlled group with 36 cases and an observation group with 36 cases. The patients in the controlled group were given Silotazole Tablets on the basis of routine lowering of sugar and lipid. The patients in the observation group were added to Bushen Huoxue Decoction,8 weeks for treatment. The paper was focus on comparing the effectiveness,observing the blood glucose and blood lipid levels,and the nerve conduction velocity and monitoring the serum homocysteine( Hyc) and the growth factor beta 1( TGF-beta 1). Results: The efficiency for patients in the observation group was 91. 7%,which was higher than that of the effective rate of the controlled group 75. 0%( P〈0. 05); The indexes of 2 hBG,FPG,Hb A1 c,TG,TC and LDL-C for patients in the observation group were lower than that of the controlled group( P〈0. 05); The SNCV and MNCV levels of nerve conduction velocity for patients in the observation group were higher than that of the controlled group after treatment( P〈0. 05); The level of Hcy and TGF-β1 for patients in the observation group were lower than that of the controlled group after treatment( P〈0. 05). Conclusion: Treating diabetic peripheral vascular neuropathy patients with nourishing kidney and activating blood methods has the satisfactory clinical effect. It is good to improve the metabolism of sugar fat and increase the speed of nerve conduction. The mechanism is related to the regulation of Hyc and TGF-beta1 in serum to reduce neuron damage,is worthy of clinical promotion.
