自拟脉通方外洗结合中医情志干预对糖尿病足病(0级)疗效观察

摘　　要：目的:观察自拟脉通方外洗结合中医情志干预对糖尿病足病(0级)的疗效。方法:采用随机对照法,选取2014年01月~2016年12月于我院就诊的0级糖尿病足患者120例,用随机数字表法分为观察组和对照组各60例。两组患者均在对症降糖、降压、调脂的基础上,对照组予一般糖尿病健康教育及温水浸泡双下肢,观察组予自拟脉通方浸泡双下肢,并同时予中医情志相胜法进行干预,采用中医证候积分、疗效评分及忧郁自评量表评分比较治疗前后变化。结果:干预后观察组的症状积分低于对照组,差异有统计学意义(P<0. 05);对照组总有效率为70%,观察组总有效率为90%,差异具有统计学意义(P<0. 05);干预后两组患者的SDS评分出现下降(P<0. 05),且两个评价指标的评分比较,观察组显著低于对照组(P<0. 05)。结论:中药脉通方外洗结合中医情志疏导,在糖尿病足0级治疗中效果较好,可有效降低足部溃疡的发生,值得临床推广。Objective： To observe the effect of Maitong Recipe external washing combined with traditional Chinese medicine emotion intervention for diabetic foot disease（ 0） patients. Methods： The randomized method,120 patients with diabetic foot 0 from Januray 2014 to December 2016 in our hospital were randomly divided into observation group and controlled group of 60 cases. The two groups of patients were symptomatic in antihypertensive lipid-lowering,hypoglycemic,based on the controlled group received general health education on diabetes and warm water for the lower limbs,the observation group treated by Maitong Recipe soaked in lower extremities,intervention and at the same time for traditional Chinese medicine emotional method,the TCM syndrome score,curative effect score and Self rating Depression Scale scores were compared before and after the treatment. Results：The observation group after the intervention of the symptom score lower than the controlled group,the difference was statistically significant（ P〈0. 05）; The total effective rate of the controlled group was 70%,the total efficiency of the observation group was90%,the difference was statistically significant（ P〈0. 05）; The intervention of the two groups of patients the SDS score decreased（ P〈0. 05）,and two evaluation indexes were compared,the observation group was significantly lower than the controlled group（ P〈0. 05）. Conclusion： Traditional Chinese medicine Maitong Recipe external washing combined with traditional Chinese medicine emotion counseling,in effect in the treatment of diabetic foot 0 is good,can effectively reduce the foot ulcer. It is worthyof clinical promotion.
