摘 要:目的通过调查唐山市14个县(市)区重点人群布鲁氏菌病(简称布病)健康教育和行为干预前后健康知识知晓率、行为形成率以及布病发病率的变化,评价干预措施的效果。方法 2014年对重点人群首先进行基线调查,之后连续两年开展布病健康教育和行为干预,对其进行相同调查表的问卷调查及血清学检测,比较干预前后健康知识知晓率、行为形成率、血清阳性率及全年布病发病率的变化。结果干预后重点职业人群布病知识知晓率(87.95%)、健康行为形成率(71.44%)分别较干预前的知识知晓率(50.87%)、健康行为形成率(45.76%)显著提高,差异有统计学意义(χ~2=2.374,P=0.000;χ~2=9.967,P=0.000),并且血清学检测阳性率及布病发病率显著下降(χ~2=22.274,P=0.000;χ~2=33.431,P=0.000),干预前后差异均有统计学意义。结论通过在唐山市重点人群中开展健康教育和行为干预,各县(市)区布病健康知识水平均显著提高,并且健康行为逐渐形成,从而布病血清学阳性率显著下降,可能是布病发病率显著降低的重要原因。Objective To investigate the changes in health knowledge,behavioral formation rates,and incidence of Brucellosis before and after health education and behavioral interventions in key populations of 14 counties( cities) in Tangshan,so as to evaluate the effectiveness of interventions. Methods Baseline survey was conducted in 2014,followed by two consecutive years of health education and behavioral intervention. Questionnaire surveys and serological tests were conducted on the same questionnaires to compare health knowledge awareness,behavioral development rates,and serum levels before and after interventions. In addition,the positive rate of serological detection and the rate of incidence of Brucellosis in 2014 was compared with that in 2015 and 2016,based on the data on the direct reporting network system. Results Brucellosis knowledge awareness rate( 87. 95%)and health behavior formation rate( 71. 44%) of target population improved significantly than that before intervention,which were knowledge awareness rate( 50. 87%),health behavior formation rate( 45. 76%) respectively( χ^2= 2. 374,P = 0. 000; χ^2= 9. 967,P = 0. 000). In addition,the positive rate of Brucellosis serological detection and the incidence of Brucellosis was significantly declined before and after intervention. The differences were statistically significant.( χ^2= 22. 274,P = 0. 000; χ^2=33. 431,P = 0. 000). Conclusion Through the implementation of health education and behavior intervention among key populations in Tangshan,the health knowledge of the disease in all counties( cities) has been significantly improved,and health behaviors have gradually developed,resulting in a significant decrease in the serological positive rate of the disease,which may be an important reason for the significant reduction in the incidence of Brucellosis.
