社区健康管理在城市社区慢性病管理中的应用效果

摘　　要：目的探讨社区健康管理在城市社区慢性病管理中的应用效果。方法选取童家桥街道1 000例慢性病高危社区居民,其中瓷器口社区500例为对照组,童家桥社区500例为干预组。对照组居民采用一般管理,干预组居民采用社区健康管理。比较两组管理前后饮食结构、运动状况、吸烟情况、体质指数(BMI)、腰围、血压、血糖、血脂达标情况。结果管理前,两组人群饮食结构、运动状况、吸烟、BMI、腰围、血压、血糖、血脂达标率比较,差异无统计学意义(P>0.05);管理后,干预组人群饮食结构、运动状况、吸烟、BMI、血脂达标率高于对照组(P<0.05)。结论社区健康管理在社区慢性病高危人群管理中的应用效果明显,其可促使社区居民纠正不良生活方式,控制慢性病影响因素,对降低慢性病发病率具有积极意义。Objective To evaluate the application effect of community health management in chronic diseases patients in urban communities.Methods A total of 1 000 chronic diseases patients with high risk community residents in Tongjiaqiao Street were selected as research object,500 cases in the Community of Porcelain mouth as control group,and 500 cases in Tongjiaqiao Community as intervention group.The control group was given general management,intervention group was given community health management.The control status of dietary structure,movement,smoking,BMI,waistline,blood pressure,blood sugar,blood fat were compared between the two groups befere and after treatment.Results Before management,no siginificant differences of the control rate of dietary structure,movement,smoking,BMI,waistline,blood pressure,blood sugar or blood fat was found between the two groups（P〉0.05）;after management,the intervention group of the control rate of dietary structure,movement,smoking,BMI,blood fat were higher than control group（P〈0.05）.Conclusion Community health management has a good application effect in chronic diseases patients in urban communities,it can promote community residents to correct unhealthy lifestyles and improve the influencing factors of chronic diseases,it is of positive significance to reduce the incidence of chronic diseases.
