极低出生体重早产儿动脉导管未闭的手术治疗

摘　　要：目的探讨极低出生体重早产儿动脉导管未闭的外科微创结扎手术的经验。方法本组7例在我院就诊的患有动脉导管未闭的早产儿,出生体重范围0.8~1.5 kg,全部为极低出生体重超产儿,手术时体重范围1.2~2.4 kg。均在出生后40天内实施了经胸膜外微创外科结扎导管的手术治疗。结果 7例患儿全部存活,随访显示导管全部闭合,生长发育明显改善。结论对极低出生体重早产儿PDA伴发临床症状者及内科保守治疗无好转者,外科微小创伤的手术治疗是安全有效的。Objective To discuss the surgical experience of minimal-invasive surgical ligation in the treatment of patent ductus arteriosus（ PDA） for very low weight premature infant. Methods Severn premature infant with PDA were enrolled in this study,their birth weighs ranged from 0.8 kg to 1.5 kg,all of the seven cases were premature with extremely low birth weight. Body weights during operation ranged from 1.2 kg to 2.4 kg,all of them received ligation by extrapleural approach through the small incision within 40 days of birth.Results All the Seven patients survived,the following results showed that all the ductus closure,with obvious improvement of growth and development.Conclusions Minimal-invasive ligation for very low weight premature infant with PDA is safe and effective,especially when conservative treatment in internal medicine is ineffective for patient with clinical symptoms.
