超声引导联合静脉留置针斜轴面在颈内静脉穿刺的临床应用

摘　　要：目的评价超声引导联合静脉留置针置管斜轴位颈内静脉穿刺置管的可行性及临床效果。方法选择需要颈内静脉穿刺置管的手术患者100例,将患者随机分为超声引导联合静脉留置针组(C组),超声联合Y型穿刺针组(P组),每组50例。记录2组穿刺次数、一次穿刺成功例数、穿刺成功例数、穿刺失败例数、穿刺时间,以及穿刺并发症的发生情况,记录操作者穿刺过程的舒适度。结果两组穿刺时间无明显差异(P>0.05),两组间的穿刺次数、一次穿刺成功例数、穿刺成功例数和穿刺失败例数的差异均无统计学意义(P>0.05);2组穿刺并发症发生率无统计学意义(P>0.05);操作舒适度方面,C组操作者舒适度更好。结论超声引导联合静脉留置针斜轴面穿刺用于颈内静脉穿刺置管,临床上安全可行,相比Y型穿刺针操作舒适度上更具有优势,值得临床推广。Objective To evaluate the feasibility and clinical effect of ultrasound guided oblique axis internal jugular vein catheterization of venous indwelling needle. Methods 100 patients scheduled for internal jugular vein catheterization were selected,and were randomly divided into ultrasound guided venous cannula group（ group C）,ultrasound guided Y puncture needle group（ group P）,50 cases in each group. The number of punctures,one-time successful puncture cases,number of puncture success and failure cases,puncture time,puncture complications were recorded,and the operators＇ comfort in the puncture course was recorded. Results There was no significant difference in puncture time between the two groups（ P0.05）. The number of punctures,one-time successful puncture cases,puncture success and failure cases between two groups were similar（ P 0.05）. So did the incidence of puncture complications between two groups（ P0.05）,but the operators＇ comfort of group C was better. Conclusions Ultrasound guided oblique axis internal jugular vein catheterization of venous cannula is simple and feasible. Compared with group P,which has more advantages in operators＇ comfort,it is worthy of clinical promotion.
