鼓膜穿刺联合鼓室灌注地塞米松治疗分泌性中耳炎效果观察

摘　　要：目的观察鼓膜穿刺联合鼓室灌注地塞米松治疗分泌性中耳炎的临床效果。方法选取2015年1月-2017年6月医院诊治的分泌性中耳炎患者84例,随机分观察组和对照组各42例,对照组单纯采用鼓膜穿刺治疗,观察组在对照组基础上使用地塞米松进行鼓室冲洗。比较2组患者临床疗效及不良反应发生情况。结果 2组均随访9个月～3年,平均(10.45±2.67)个月。观察组总有效率为95.24%高于对照组的80.95%,差异有统计学意义(χ~2=4.08,P <0.05)。对照组出现短时间一过性耳内堵塞感4例,观察组出现短时间耳内堵塞感3例,出现眩晕、恶心感2例,均自行恢复。结论鼓膜穿刺联合地塞米松鼓室注射治疗中耳炎的治疗效果较好,安全性较高,值得临床推广。Objective To compare the clinical effects of tympanic membrane puncture combined with tympanic cavity perfusion of dexamethasone in treatment of secretory otitis media.Methods 84 patients with secretory otitis media,selected from January 2015 to June 2017 in the hospital,were randomly divided into observation group and control group,each of 42 cases.Control group was treated with tympanostomy alone,and observation group was treated with dexamethasone for tympanic washing on the basis of control group.The clinical efficacy and adverse reactions of two groups were compared.Results Both groups were followed up for 9 months to 3 years,with an average of（10.45 ± 2.67） months.The total effective rate of observation group was 95.24% higher than 80.95% of control group,and the difference was statistically significant（χ^2= 4.08,P〈0.05）.In control group,there were 4 cases of transient occlusion in the ear,and 3 cases of occlusion in the ear in the shortterm,2 cases of dizziness and nausea.All recover themselves.Conclusion The effect of tympanic membrane puncture combined with dexamethasone tympanum injection in treatment of otitis media is significant and safe,and it is worth popularizing in clinic.
