摘 要:目的:采用弥散峰度成像(DKI)对2型糖尿病认知损害的评价价值。材料与方法:2型糖尿病患者40例纳入糖尿病组,受教育程度、年龄相匹配的非糖尿病患者40例为病例对照组,均行DKI扫描、认知功能测评(MMSE评分),测量两组各感兴趣区DKI指标,观察有无明显差异。并对存在组间差异的DKI指标与MMSE评分之间进行相关分析。结果:与对照组对比,糖尿病组:大部分感兴趣区MK、AK、RK值降低(P<0.05)。右额叶、右海马、右半卵圆中心的FA值降低,AD值升高;右海马及胼胝体压部RD、MD值升高;部分感兴趣区MK、AK、RK及FA值与MMSE评分呈正相关,AD值与MMSE评分呈负相关。胼胝体压部及右海马MD值与MMSE评分呈负相关。结论:DKI既能提供传统的弥散指标,又能提供额外的峰度指标,可用于检测2型糖尿病患者脑部超微结构,为患者认知功能损害评价提供客观指标。Purpose: To explore the the value of Diffusion kurtosis imaging( DKI) in type 2 diabetes mellitus( T2DM). Materials and Methods: DKl were applied to 40 patients with T2DM and 40 patients without T2DM to be the control group,and get their MMSE scores. Measure the value of DKI indexes in bilateral ROI of T2DM and control group,compare the mean values of DKI indexes between the two groups,and correlate the mean value of DKI indexes with MMSE scores. Results: Compared with control group,the mean MK、AK、RK values of most of ROI in T2DM group decrease( P〈 0. 05). The mean FA value of right frontal lobe,right hippocampal,center of right hemi oval in T2DM group decrease,but the mean AD value increase. The mean RD、MD value of right hippocampal,pad of corpus callosum in T2DM group increase. The mean MK、AK、RK of parts of ROI in T2DM group were positive correlated with MMSE scores,but the mean AD of them were negative correlated with MMSE scores. The mean RD、MD value of right hippocampal,pad of corpus callosum were negative correlated with MMSE scores. Conclusion: DKI can not only provide traditional dispersion index,but also provide additional kurtosis index. It can be used to detect the ultrastructure of brain in patients with T2DM,provide objective index for the evaluation of the patients' cognitive impairment.
